My proudest moment as a nurse didn't involve anything dramatic--no blood, no CPR, no crash carts. It involved asking clients a single question. When I worked in an immunization clinic, one of the questions on the screening form was, "Have you had anything to eat in the last 4 hours?" This question was used to determine if someone was likely to pass out during the visit. But that isn't the question I'm talking about. It was very common for people to answer no to that question. My colleagues and I would provide some juice and crackers, then administer the vaccines. But I noticed that, quite frequently, people were marking no to that question well within 4 hours of breakfast or lunch. What was going on here? Because many shots are given in a series, it was possible to track clients' answers over time and see that the same clients would answer no over multiple visits. This data suggested that they regularly skipped at least one meal. So I decided to add a question to our screening form: "Would you like information about local food pantries?" Lo and behold, fully 1/3 of our clients marked yes to that question! Because of the changes to our screening form we were able to create a touchpoint in the healthcare system where people experiencing hunger could get resources during a visit for routine care. The clinic had been collecting data that pointed to a problem for many years, but had never taken the extra step to zoom out and analyze what it meant. The result was a simple intervention. A single question--one checkbox. It resulted in the distribution of a sheet with various programs, food boxes, and pantries within the community. The time spent was something on the order of 30 seconds. But it made a real difference to people and families who were struggling. And I will forever be proud that such a simple effort could yield such amazing results. Conversely, the non-vaccinated group was disproportionally made up of Black, Hispanic, dually eligible for Medicare and #Medicaid, and low-income subsidy (LIS) patients.
