Expands First Nations Health Services with 13 New Primary Care Centers, 10 Opening This Fall "British Columbia is launching 13 new First Nations primary care centres, with 10 set to open this fall. The First Nations Health Authority and Health Ministry announced that, once all centres are operational, B.C. will have a total of 15 such clinics. Health Minister Adrian Dix stated this expansion is part of efforts to eliminate Indigenous-specific racism in the province's health-care system. The centres will provide integrated primary care, social services, and Indigenous health supports, operated in partnership with local First Nations. Facilities opening this fall include the Coast Salish, Gitxsan-Wet’suwet’en, and Nuxalk Primary Care Centres. As always, the key is primary care and, despite at least 30 years worth of strategies to "shift left", the proportion of spend that has gone into primary care has reduced (it was 25% in 2010/11 and only 18% in 20/21). Unsustainable primary care means more demand in secondary care not less, yet how many care provider boards focus on investing in primary care? To illustrate, between 2016 and 2022 WTE qualified GPs fell by 22% while hospital consultants grew by 25%. Until commissioners of care/integrated care providers change their view of, and relationship with, primary care, seeing them as a key part of service provision rather than an inconvenience, this won't change. The NHS Confederation in 2023 suggested that reducing acute care demand through increased primary care spending generates a significant return on investment (31%), but I'd be willing to bet that there are few plans to re-balance the proportion of spend away from acute care to any great degree in current plans, and equally willing to bet that primary care is not at the forefront of reform plans when they arrive. If this doesn't happen, i.e. a proper system based approach, there will be little real change I suspect, just more fine words.

Renewal and reform of the NHS seems to be on the cards, and we hard talk over the weekend of the need for separation of acute and elective services, investment in infrastructure and investment in technology. What is needed more perhaps is a dialogue with the public that the District General Hospital model that comes from the 1960's is dead - a hospital for every community is neither affordable or desirable from a clinical perspective. Every health community I've worked with on service change has grappled with getting that across to the public. Leadership at a national level would help. Local politicians of all stripes are one of the main barriers to health communities that try and drive forward even appropriate and well thought through change. I want the best quality care that I can get in a timely manner. It doesn't need to be on my doorstep - that is what primary care is for.

The top priority for 2025 is advancing our prospective lymphedema screening initiative for our patients. In hindsight, it seems so obvious to focus our resources on this, yet we are the first hospital in Singapore to actively identify high-risk patients and screen them using a multimodal approach, including ICG lymphography. On the topic of firsts, our team has also secured national ethics board approval and funding to launch our trial on extracranial lymphatic surgery for the treatment of Alzheimer's disease—a groundbreaking start to the year. With Alzheimer's becoming an increasingly prevalent issue in Singapore's aging population, current medications offer only limited benefit, slowing disease progression by mere months. If successful, this trial could provide a much-needed glimmer of hope in what has long felt like a hopeless battle. Great to be pushing ahead with teammates like Vincent K.S. Tay Kok Pin Ng and Lim Si Ching. Not forgetting our other colleagues from neurology, Anaesthesia , neuro radiology, nuclear med.

Last week's Health Inequalities Conference was fantastic. A day of insight, education, inspiration, plotting, progress and achievement. Over 160 people attended. Attendees said their main takeaway from the conference was how much work is already in motion and they welcomed being given an insight into the work on inclusivity. Director of Nursing, Allied Health Professionals and Quality, Judith Douglas, said: "Tackling inequalities in access to health care is a key priority for CityCare... the importance of engagement and co-production, involving patients and the public to help us design and deliver services that are accessible to those who are often among the most vulnerable in our community." Community models within basic public services like EMS and paramedicine provide a strategy that is working across the nation to address non-medical drivers of health. Greater Longview Optimal Wellness (GLOW) is a part of EHF's Texas Accountable Communities for Health Initiative (TACHI). TACHI is changing the way Texas thinks about health – from one that is centered on the health care system to one that values health equity and multi-sector partnerships. GLOW is a great example of how EMS efforts can team up with community organizations to use innovative strategies to address non-medical factors. The goal? Reduce ER overuse and improve health outcomes for those in Longview who face serious non-medical barriers to good health like housing, food insecurity, and more.
