This morning our practice hosted 20 6th form students with an interest in future medicine/healthcare roles. For my part, I spoke about my experiences of the front line paramedic role, my transition into primary care and the new pathway to advanced practice. I thought it was important to highlight the fact that A level results don't always define your future, as my A level results below demonstrate. We also discussed resilience, burnout and prioritising your own health. 👨🏻⚕️ What I found out about myself today dealing with back to back acute and complex cases in a fully booked GP clinic topped with needing to send 3 people to A&E and call a blue light ambulance for a very poorly patient right at the end of a very busy, almost ten hour day: ❌I don’t like acute situations when it comes to human life ❌ Period. No human being can safely deal with life and death situations at the end of a day demanding your 100% every minute right until the last minute of a 10 hour day. I’d much rather do the chat, meds review, shared decision making, holistic and wholesome Family Doctor experience, which GP in the U.K. originally was at one point. I feel like I’m on a combination of AMU and AMU-Lite shifts most of the time. The acuity and complexity in General Practice has never been this bad in recent years. Not every medic needs to be an ‘acute’ medic especially when GP was meant to mainly deal with things that are chronic. "Fix general practice – you fix the NHS" say the BMA's General Practitioners Committee. A bold claim....and much needed challenge to the secondary care-centric NHSE status quo. Whatever you want, GPC... (oldies will get it 😀) This excellent 5-page report, produced to support the Labour Government in its desire to re-prioritise General Practice, is well worth a read. I like this bold summary on the report's final page, which I wholeheartedly agree with: "We need to commit to transform the NHS over the next decade into a home-first, community-second, admission-last model which prioritises prevention and builds back preventative care through expert-generalist GP-led general practice." A big ask for Wes Streeting but all indications are that its a challenge he is willing to face into. Whilst the GPC doesn't explicity mention us, I'm sure they would love to involve the army of Patient Participation Group members around the country working alongside their GP practices to ensure any resulting reforms are co-produced to ensure maximum engagement and benefit to patients.... #NHSrecovery #primarycare #rebuidingtheNHS #patientengagement #coproduction #NHStransformation Vin Diwakar

Ensure falls are correctly documented and removed if no longer applicable. Verify that residents on psych medications have the appropriate diagnoses. Ensure wounds are accurately captured. Capture Everyone in Your PBJ: Conduct an internal audit to find any missing staff, including medical directors and agency staff. Make sure every hour is accounted for to avoid discrepancies. Special shout out to the facilities investing in improving their star levels – your dedication makes a huge difference! A few years ago, I published an article on KevinMD.com about the “Hippocratic Hypocrisy” that I see and experience. 3 years later, I can’t say that I’ve seen or heard about much for tangible change on the system level. Is this frustrating? YES! Is it likely to improve anytime soon? DOUBTFUL. Of course limiting the amount of GP appointments available in a given day is going to cause harm. Many people already struggle to see a GP in good time - if at all, leading to worsening health and over-reliance on emergency departments. But expecting GP's to carry out huge amounts of unpaid work is not going to help either, especially as this leads to increased stress. We need to support GP's in their call for fairer and less exploitative working conditions. GP wellbeing IS patient safety. But we also need to consider what else can be done to improve how people access GP services - and what they can expect to get out of them Better use of 111 services and pharmacies for minor ailments is already underway. And more and more GP's are using AI and automated tools to assist with note taking, admin, and routine tasks. Yet we believe that the biggest opportunity lies in rethinking how people approach appointments and communicate with GP's in the first place. Some of this can be done through the new NHS roles: General practice assistants and health and wellbeing coaches can do more to signpost people to other services for example. But we are yet to see this happen at scale, and we also need more assurance that these new roles will not replace valuable GP services or undermine them. We also need to consider how healthcare can do more to help people with disabilities, and more complex and chronic health issues - who already find GP services difficult to use, or who struggle to be taken seriously at all. That's why we've come up with Health Heard, and the health check-in. This new service supports people to identify, record and share what's going on for them and their health in a different, more structured way. People can bring Health Story reports to an appointment when they have a lot that they need to talk about, and there is not a lot of time, or if they are concerned that something is being overlooked. This saves valuable time, and means that more of the appointment can be used for assessment and next steps. We also provide guidance and additional support to help people to communicate their concerns more effectively - and in a way that respects GP's capacity and expectations. * Better preparation, means better appointments, and better outcomes * Interested in finding out more? A Good Faith Exam is a medical evaluation conducted via a telehealth appointment by a nurse practitioner or physician assistant. This is a necessary step to stay compliant with California regulations. It is a simple, quick appointment. They generally ask most of the same questions as asked on your intake form. Do I have to do this step? Yes. Mobile IV Wellness has always strived to do what is safest for our clients as well as to stay compliant with state regulations and medical board regulations. We do know that it may seem pointless for some, but it absolutely is a necessary step. We use a company called SpaKinect. Generally, these appointments will be completed at your appointment, but they also can be done ahead of time in the comfort of your home on your time. All new clients must complete a Good Faith Exam. No exceptions. If for some reason you believe you need an appointment during off hours, please let us know ahead of time.
