Every wondered what a pulmonary embolus looks like on dynamic PET/CT? It is almost a year since I first rang a referring clinician to tell them that their patient had a PE and was greeted with the question as to whether we had done a VQ scan. Since then I have explained on a number of patients, the visualisation of multiple segmentation perfusion abnormalities on the first pass of the radioactive bolus through the lungs in the absence of clear reasons for this on correlative CT. At the Melbourne Theranostic Innovation Centre we are learning the advantages of "all organ" dynamic imaging as we go. In this case the region of interest wasn't the chest and would never have been included in dynamic imaging on a conventional field-of-view scanner. At least in Australia, there is no reimbursement for doing other than a delayed "whole body" scan which typically includes only vertex to upper thighs. We feel that there are major benefits to patient care in doing dynamic imaging in many clinical situations, including evaluation perfusion of lesions, their relation to vascular structures, particularly in patients with contraindications to contrast and for pharmacological interventions like diuretic renograms. "When we think we know all the answers, we stop looking for them."   -      Eric Barker, from "Barking Up the Wrong Tree: The Surprising Science Behind Why Everything You Know About Success Is (Mostly) Wrong" As our experience with dynamic "all organ" imaging grows, we continue to question how and when to best incorporate it into our clinical protocols to provide the optimum convenience and diagnostic outcomes for our patients. Approaches to improving detection of prostate bed recurrences through early imaging (See: Uprimny, C. et al. EJNMMI 2017;44:1647–55) or forced diuresis (See: Uprimny, C. et al. J Nucl Med 2021; 62:1550–57), provide options for conventional field-of-view scanners but still aren't widely used, seemingly for workflow reasons. We think the ability to compare kinetics of uptake or clearance of activity may help to better characterize abnormalities outside the pelvis as well as a plethora of physiologically and pathologically relevant findings not apparent on delayed imaging. However, I admit, we are still learning and have many questions still to answer. In the case below, anastomotic and right pedicle recurrences are seen very early in a 10-minute dynamic series reconstructed as 1-minute frames, but difficult to  assess on delayed imaging while a pulmonary metastasis has the same kinetics as the prostate bed disease, increasing confidence in its nature.

💡 New findings published in the American Journal of Cardiology show patients with persistent atrial fibrillation (AFib) who undergo transcatheter aortic valve replacement (TAVR) may face a higher risk of long-term mortality. 🤔 Yet, this doesn’t appear to be the case for patients presenting with preoperative paroxysmal AFib. These findings were based on a review of data from nearly 3,000 TAVR patients at a single high-volume medical center in the U.S. 🏥 A year after TAVR, AFib patients experienced higher rates of all-cause mortality than those without AFib. After a mean follow-up period of four years, however, the study authors noticed a trend: there was no longer an increased risk of all-cause mortality among the group with preoperative paroxysmal AFib, while preoperative persistent AFib was still associated with a greater mortality risk. 🫀 Researchers suspect the overlap between AFib risk factors and aortic stenosis, including advanced age and heart disease, may be an explanation for their findings. #cardiacsurgery #cardiothoracicsurgery #ctsurgery #tavr 🔗 Check out the full story:
