The news of Walgreens closing around 1,200 stores is yet another reminder of the shifting landscape of healthcare access in America, especially in rural communities. For many in rural areas, the local drugstore isn’t just a place to fill prescriptions—it’s often the closest thing to a healthcare hub. The closure of these stores could widen the healthcare access gap, especially for managing chronic conditions like diabetes or hypertension, where consistent, in-person support can make a difference. However, this trend also signals a moment for transformation. As large retail chains pull back, we need to rethink healthcare delivery models that are community-driven and technology-enabled. Mobile health units, telehealth platforms, and community health workers could fill the void left by these closures. This is where partnerships and innovative deployment strategies, like what we’re developing at The Rural Health Technology Consortium, can make a real impact. It’s time to bring healthcare to where people are—whether through digital solutions or local, on-the-ground efforts. The path forward requires reimagining the role of technology in rural health, but one thing is clear: We cannot afford to let these closures leave our communities without support. The venture faced challenges with billing, leadership changes, and operational hurdles. Why are Walmart, Walgreens, and CVS struggling with their retail health clinic experiments? In-store healthcare clinics, once hailed for their convenience, have faltered, with Walmart and Walgreens closing numerous locations. CVS's MinuteClinic, though popular with some patients, faces closures in several regions. The failure stems from thin profit margins and unforeseen challenges, according to Timothy Hoff, a healthcare management professor. These clinics, once seen as the future of primary care, struggle with low reimbursements, high expenses, and staffing issues. The clinics rely on high patient volume yet struggle to turn a profit. Cross-selling strategies also failed to materialize, as sick patients are unlikely to buy unrelated products. Moreover, staffing shortages and insufficient training further hindered their success. CVS, Walmart, and Walgreens have all faced setbacks, signaling a broader struggle to reconcile healthcare with retail business models. Changes are coming to how Star ratings are measured for #MedicareAdvantage plans. According to a recent McKinsey report, there will be a shift towards reweighting member experience measures, putting more emphasis on clinical and pharmacy metrics. This means MA plans will have to step up their game in those areas to maintain those coveted 4+ Star ratings. MA plans might combine in-person care with virtual care strategies to deliver whole-person care while maintaining the same quality both virtually and in person, since fragmented virtual care experiences are expected to lower Star ratings. Look out for #healthinsurance plans offering more #telehealth services in the near future to keep their Star ratings high. It should be top notch, too: Lackluster virtual services will render the additions a moot point.

I can understand why retail giants have pursued primary care--(1) new revenue streams, (2) additional boosts to lagging store sales, and (3) low cost equipment, staffing, and overhead costs. Makes sense. But the economics of fee-for-service primary care in the US can be challenging. Large health systems can use primary care to control patient flows to their facilities and also drive more lucrative specialty care. I don't see the same upside for the retail giants as their ability to leverage primary care to support other service lines through vertically integrated services is limited relative to traditoinal heatlhcare. Hence, we hear of WalmartHealth announcing the closing of 51 stores. #Cicero #healthcare
