In 2024, the Institute for Safe Medication Practices (ISMP) has updated their list, drawing insights from practitioner feedback, clinical literature, and expert input. Notable changes include the addition of tranexamic acid, due to its neurotoxic properties when administered via the wrong route. Reducing errors with these medications requires safeguards such as limited access, clinical decision support, standardized procedures, double checks, and improved information access. CDC is updating its Expanded Access-Investigational New Drug (EA-IND) protocol to clarify eligibility for TPOXX, the antiviral drug used to treat mpox.
