"Number of free-standing ERs grew by 10X from 2001 - 2016" This is a perfect microcosm of what is #broken in our healthcare system today. While there may be value in having access to a free-standing ER (not sure what that is and probably more indicative of loss of hospitals in general in a region), the end result is #consumer confusion, increased billing (i.e. health system revenue), and patient frustration.

When it comes to patient safety in home health care, being prepared for emergencies is crucial. One simple but essential step is ensuring that the patient and their caregivers have quick access to a comprehensive list of emergency contacts. This list should include: - Family members or designated emergency contacts - Doctors and specialists - Local emergency services (911, police, fire department) - Home health agency or care manager - Pharmacy or medical equipment providers Create a clear, concise list with full names, titles, and phone numbers. Display it prominently in the patient's home, such as on the refrigerator, and keep copies near their bedside or with their primary caregiver. Consider storing the information electronically for easy access. Regularly review and update the list, especially if there are changes in the patient's condition, medications, or care team. Having this vital information readily available can save valuable time in an emergency situation. By keeping emergency contacts close at hand, you provide peace of mind and help ensure a swift response when it matters most. It’s a pleasure to share with you this 20min documentary which is the first ever close up view of the Singapore emergency medical services from dispatch, ambulances to the emergency department. It’s really nostalgic to see how 995 has been transformed, EMS has changed so much since I stepped in 13 years ago into 995 as an MOH visiting consultant initially and then taking over as the Director of the Medical Department. EMS has grown from strength to strength with each successive EMS director and CMO. It’s however still challenging that last year we went from 213,615 calls in 2021 to 256,837 calls in 2022, an 18% increase. As you can imagine, capacity does not increase at that rate. We have 10,000 non-emergency calls which is remarkably low already for a emergency service that is free to all callers. What then is the next step? I believe that telehealth and digital health are the next transformative leap that is needed to support citizens who potentially have acute conditions but are unsure if they need to call 995. Potentially there is at least 20% or more of patients that are P3 (lower priority) that potentially could be self-managed, treated in the community by general practitioners or maybe even community organisations. There is a wide range of health issues that potentially can be better managed by having a self-service health chatbot services that can help answer common health queries and complemented by a national nursing helpline to solve more complex issues with support from their medical records, available in the National Electronic Health Record (NEHR) system. If this is paired with access to booking/expediting medical appointments there are many ambulance rides and ED visits that can potentially be avoided. There are situations where patients need to go to secondary or tertiary care facilities but do not need an emergency ambulance. Some systems in Australia give out taxi vouchers to such cases to avoid the use of limited emergency ambulance capacity.
