Sepsis is frequent and an emmergency. But what is sepsis? 🔥Sepsis is the body's "desperate" response to an infection it considers threatening. The Journal of the American Medical Association (JAMA) recently published findings from a study titled “Delays to Antibiotics in the Emergency Department and Risk of Mortality in Children With Sepsis.” This retrospective cohort study utilized data from the Improving Pediatric Sepsis Outcomes (IPSO) collaborative, which includes 51 US hospitals and 19,515 children aged 29 days to less than 18 years. The study provides new insights into the critical issue of antibiotic timing and its association with sepsis-related mortality in pediatric patients. Tackling the Recurrent UTI Challenge in Older Women: Balancing Treatment and Risks Managing recurrent urinary tract infections (rUTI) in older women is a complex and challenging task for physicians. With rUTI occurring at least twice in 6 months or three times a year, it’s twice as common in women over 65. The diagnostic dilemma arises because classic symptoms such as acute dysuria, urgency, and suprapubic pain can be misleading due to higher rates of chronic genitourinary symptoms and asymptomatic bacteriuria. Urine tests can be deceptive, often reflecting bacterial colonization rather than true infection. This makes it crucial for clinicians to assess symptoms thoroughly and avoid over-reliance on test results without symptomatic evidence of UTI. Treatment decisions are further complicated by the potential adverse effects of antibiotics, including drug interactions and the risk of antimicrobial resistance. While short (3-6 days) and long (7-14 days) antibiotic courses show no significant difference in efficacy, the decision to start antibiotics should be weighed carefully, considering the patient’s overall health and risk factors. Prevention strategies like long-term antibiotic prophylaxis and alternative methods such as vaginal estrogen or methenamine also come with their own sets of challenges and limited evidence. Physicians face a common dilemma: balancing the need for effective treatment against the risks of overprescribing antibiotics. Emphasizing a tailored approach that includes thorough symptom assessment, cautious use of antibiotics, and considering non-antibiotic options is essential for optimal management of rUTI in older women. #Healthcare #UTI #Antibiotics #Geriatrics #PatientCare

Signs and symptoms are described that we won't find early on, but we have to anticipate and learn to decode some that are hidden. Rare diseases typically affect patients from birth, involve multiple organ systems, are severely disabling, and can reduce life expectancy and impair physical and mental abilities. Between 70% and 75% of these diseases have exclusively pediatric onset. Two out of three rare diseases appear before 2 years of age, causing 20% of infant mortality and 10% of pediatric hospitalizations. Diagnostic delays and repeated consultations with various doctors before identification are common. In industrialized countries, the diagnosis of these diseases takes between 5 and 10 years, and in low-income countries, up to 30 years or more.

📃Scientific paper: Neonatal Staphylococcus Aureus Sepsis: a 20-year Western Australian experience Abstract: Objectives The purpose of this study was to characterise neonatal Staphylococcus aureus (SA) sepsis in Western Australia (WA) between 2001 and 2020 at the sole tertiary neonatal intensive care unit (NICU), examine risk factors for sepsis in the cohort, and compare short- and long-term outcomes to control infants without any sepsis. Methods Retrospective cohort study at the Neonatal Directorate at King Edward Memorial Hospital (KEMH) and Perth Children’s Hospital, using electronic databases and patient medical records. Results The overall incidence of SA sepsis was 0.10 per 1000 live births (62/614207). From 2001 to 2010 the incidence was 0.13/1000 live births, reducing to 0.07/1000 live births from 2011 to 2020. SA was most frequently isolated from endotracheal aspirates, and infants with SA sepsis had longer median duration of ventilatory support than those without any sepsis (31 days vs 18 days respectively, p < 0.001). In our cohort, SA sepsis was associated with worse neurodevelopmental outcomes compared to infants without any sepsis. Conclusions The incidence of neonatal SA sepsis has reduced over the last 20 years, suggesting potential effectiveness of the preventative interventions implemented. Endotracheal tube (ETT) colonisation and prolonged ventilation may be under-recognised as potential sources of SA infection. Our study suggests SA sepsis may negatively impact neurodevelopmental outcomes. These results point towards a high effectiveness of nirsevimab in preventing severe RSV bronchiolitis in infants requiring PICU admission from 75.9% (48.5–88.7) to 80.6% (61.6–90.3) depending on the assumptions. These real-world estimates are in line with the efficacy observed in clinical studies: the efficacy of nirsevimab was evaluated in clinical trials against very severe medically attended RSV lower respiratory tract disease between 64.2% (−12.1–88.6) and 87.5% (62.9–95.8) depending on gestational age [6, 11] and at 75.7% (32.8–92.9) in a pragmatic trial. This strong effectiveness is also consistent with a Spanish study looking at RSV-related hospitalizations and French surveillance data showing a reduced impact of the 2023–2024 bronchiolitis epidemic on hospitalizations of infants under 3 months compared to previous years and in contrast to older infants. xavier saez-llorens Enrique Chacon-Cruz
