In maternal alloimmunization, MCA doppler ultrasounds are initiated if antibody titers reach a critical threshold; 1:16 for most antibodies except Kell which is critical at any level OR in a pregnancy where the mother has had a previously affected baby.
In TAPS, MCA doppler ultrasounds should be initiated every 2 weeks, alongside the routine fluid checks for TTTS. It’s important to remember that TAPS and TTTS are different diseases, and TAPS presents with no other signs, except in advanced stages. Bi-weekly (every 2 weeks) ultrasounds are recommended from 16 weeks in most countries, and this should include MCA Doppler, DVP for both twins, and umbilical artery flow direction recorded.

Low birth weight has been linked to an increased risk of future diseases, including cardiovascular disease. To address this concern, the Child Health Equity Center champions Kangaroo Care as a low-cost intervention, promoting direct skin-to-skin contact between parents and newborns. This approach is especially valuable in low-resource settings where access to advanced care in a Neonatal Intensive Care Unit (NICU) is limited.
What makes Kangaroo Care even more compelling is the strong evidence supporting its short-term benefits, such as reducing neonatal mortality and fostering parental bonding. Importantly, Kangaroo Care is now recognized as both feasible and beneficial not only in NICU settings but also in high-resource environments.
Despite this evidence, Kangaroo Care is often considered an auxiliary rather than an essential intervention for premature and low birth weight newborns. However, it's time to rethink this perspective and acknowledge the significant impact Kangaroo Care can have on the well-being and development of these vulnerable infants. Approximately 7.5 per cent of babies in the UK are born prematurely, according to the Office of National Statistics. Incubators provide a safe and secure environment for preterm babies as they recover and develop, but what should you consider when designing one? Here, Dave Walsha, sales manager at drive system supplier Electro Mechanical Systems Ltd explains.
Typically, premature babies will require more care than those born at full term. As their lungs are still underdeveloped, preterm babies will often suffer from breathing issues. Commonly, this presents itself as newborn respiratory distress syndrome (NRDS), a condition found in around half of all babies born between 28 and 32 weeks of pregnancy. Babies with NRDS will struggle to keep their lungs inflated independently, making breathing difficult.

The Heart as a Mirror:
A routine anomaly scan may underestimate the fetal heart especially when there is presence of
repeated pregnancy losses even in young parents. A cardiologist driven analysis of heart unmasks not only the progression from mild to severe fetal cardiac disease. It also unmasks latent autoimmune maternal disease.
As the presentation to cardiology department was late, we could compare previous images of USG with current echocardiography, sigh and try to fight the fire.
A lesson again that heart is the clearest mirror.
