It is characterised by excessive, coarse trabeculations within ventricular myocardium beyond papillary muscle insertion which communicates freely with the ventricular cavity. This carries high risk for development of serious arrhythmia, thromboembolic stroke, pump failure etc. Isolated LV involvement is the most common presentation (68%) and biventricular involvement is the next most common presentation (22-38%). Isolated non-compaction of RV myocardium is a rare anomaly. We hypothesized the Bezold-Jarisch reflex as the mechanism which leads to bradycardia and hypotension with syncope related to prolonged tachycardia. Our experience highlights this method as an effective therapeutic intervention for symptomatic HOCM patients, providing relief from symptoms and enhancing cardiac function, reducing Left Ventricular Outflow Tract (LVOT) gradient. However, potential complications such as arrhythmia, Left Anterior Descending (LAD) dissection, cardiac tamponade, and coronary artery spasm are rare but necessitate careful patient selection and vigilant post-procedural monitoring. The analysis allowed a systematic variation of the transluminal pressure difference and plaque thickness. Included in the analysis was a sheath of surrounding tissue that provides support during the cyclical deformation. The analysis incorporated progressively thicker layers of plaque to quantify their impact on the artery compliance. It was found that the presence of plaque plays a significant role in reducing the compliance. Giant coronary artery aneurysms are rare, with a reported prevalence of 0.02% to 0.2% [1]. Cardiac Sarcoidosis (CS) is often not recognized as an antemortem, as sudden death may be the presenting feature. Cardiac involvement may occur at any point during the course of sarcoidosis and may occur in the absence of pulmonary or systemic involvement. Sarcoidosis can involve any part of the heart.
