I have posted on here before about closure techniques for #GPOEM for #gastroparesis. While we still have much to learn about this procedure (and this video shows the end of another variation: the lesser curve or superior approach myotomy), it is clear that we are helping people to recover function and quality of life. Secure wound closure to prevent leaks and other complications remains a challenge. After waiting what seems like a long time (for obvious reasons Boston Scientific 😜), I believe I have finally found my personal favourite. I was pleased a few weeks ago to be the first in the UK to use the new #Mantis clip and it did exactly what I was hoping! Although this video doesn't seem like we are really asking a lot of the clip itself (it can do more!), the pay-off in terms of cost effectiveness and security (in terms of me faith in the device) is huge!! CLip #2 is my favourite - look how the 'fangs' of this clip really bite into the tissue to bring the edges of the mucosal incision together from so far apart. (Apologies for the non-linear finish for clip #3 ... I will try harder next time 😁). Look out for a partner video or two to this one, where we used the innovative X-tack (which is the closure I thought I was waiting for before Mantis ... and before X-tack, Overstitch!). It is wonderful to have three such powerful tools for tissue apposition and wound closure, all from Boston Scientific.
