When I started out in dentistry 6 years ago I had no clue about the links between periodontal disease and cardiovascular disease, diabetes and even pregnancy risk. No one ever explained to me the increase in overall health risks created by not having a healthy mouth. No one ever explained to me that perio is a disease, and once you have it you can only mange it, not cure it! Once I understood these risks I bought an electric toothbrush, waterpic, and started flossing more regularly and thoroughly. The numbers are staggering with >50% of adults >30 estimated to have some form of periodontal disease. Even more staggering is the fact that while the majority of adults in the US have periodontal disease, only a small % of adults in the US are in perio related programs. From my experience looking at data from over 1,000 dental practices, I have seen many practices with <2% of patients over 30 in perio related programs (D4910), and on average < 15 % of adults in perio related programs. Why is the diagnostic and treatment rate of periodontal disease so much lower then the incidence rate? There are a myriad of challenges with diagnosing and treating periodontal disease including: - Do we have clear clinical standards in our practices about how we diagnose Perio? - Are we measuring perio visit / diagnostic and acceptance rate by location and provider? - Are we regularly perio probing and recording results? - Are providers trained and comfortable having the difficult conversation with patients informing them they have a disease? - Are we communicating clearly about the risks associated with the disease? - Do we have time blocked in provider schedules to accommodate more lengthy perio scaling procedures? - Are we doing bloody prophies to instead of telling them they have a disease? - Are treatment planners working to remove barriers to acceptance, like cost?
