What is your approach in SCAD patients? Is the invasive approach always necessary? This is a 34-year-old patient, postpartum period, admitted to our department for self-resolved precordial pain and V1-V4 negative T waves on EKG. At first check rapid hemodynamic deterioration due to total occlusion of anterior descending artery. Although invasive coronary angiography is the gold standard for the diagnosis of SCAD, especially when combined with intracoronary imaging, it is associated with considerable risk of dissection propagation. When clinical data are strongly suggestive of SCAD, is it reasonable to proceed with CT coronary angiography and prevent major complications? What is your routine? Do you use CT coronary angiography only for follow-up or also for diagnostic purpose?
