The resolution! We all want it! But yet a lot of articles and advice from management is: don’t state me problems, give me resolutions! So how you find the resolutions without the problem statement that usually needs to be defined through discussions? Discussion about the problem is just the process for its resolution! It is preventative method! It can be compared to hazard reporting! So why so many of us resist the problem statement and discourage our employees to bring it up or simply ignore???

When we first engage with new participants we must always be considering safety through a safeguarding lens. Right first means - we meet persons need, make modifications and ensure a non aversive approach to managing risk. We can’t manage safety by using strategies or restrictive practices that place people’s physical and psychological safety at risk. Having a policy in place can be your go-to playbook when navigating a crisis. However, it's essential to keep it current through regular refreshing and testing. Another crucial aspect is recording your decisions and thought process during an incident. Remember, the faintest ink is better than the brightest memory. As the saying goes, no one remembers when you get it right, but no one forgets when you get it wrong. So, take the time to ensure your policy and procedures are up to date and your team is prepared to handle any situation that may arise.

Right, The 5 Whys methodology is indeed a valuable approach for identifying the root causes of problems or incidents. By repeatedly asking “why” to delve deeper into the issue, it can reveal underlying causes that might otherwise be overlooked. This thorough analysis can lead to more effective solutions and preventive measures in the future.
