This morning, A female patient arrived for a pre-THR surgery consultation. She weighed 295 pounds. She was 58 years of age and was 5'6'' tall. Her BMI was off the scale; the examination confirmed she needed a THR. What to do... I knew that my anaesthetist college would not be that excited about going ahead with the procedure, with all the extended potential risks and possible complications. She needed her to lose weight; any amount would be advantageous, but ideally, it would be beneficial to see a reduction of at least 50 pounds. We know that anything that involves exercise would not be suitable as the pain in the hip is extreme. She was directed, with a bit of persuasion, to download and complete the Oxford Therapeutics psychology-focused weight loss programme. Are you encountering an evergrowing surge in obese patients? It takes a lot more time to counsel these patients appropriately and explain why they need to drop some weight to optimize outcomes. If their BMI is up to 45 and they have no other risk factors for infection (diabetes, smoking, immunosuppressed) then I will proceed with surgery after ensuring they are not protein malnourished. If they are over 40 and have other risk factors then I give them a target weight and recheck on them in 2-3 months. If they don’t demonstrate weight loss at that point I refer them to their GP. #ozempic might be a solution, I just saw a patient with 25kg weight loss in 3 months prior to THR.
