Natural Killer (NK) cells combat cancer by inducing programmed cell death (apoptosis) and cell injury death (necrosis) in tumor cells. They trigger apoptosis by engaging death receptors on cancer cells or releasing cytotoxic substances like perforin and granzymes, which disrupt the cancer cell's membrane and initiate the apoptosis pathway. NK cells can also cause necrosis by directly damaging the cancer cell's membrane or through inflammatory responses, leading to cell rupture and death. The major criteria for the syndrome are polyradiculoneuropathy, clonal plasma cell disorder (PCD), sclerotic bone lesions, elevated vascular endothelial growth factor, and the presence of Castleman disease. Minor features include organomegaly, endocrinopathy, characteristic skin changes, papilledema, extravascular volume overload, and thrombocytosis [1]. Autologous Stem Cell Transplantation (ASCT) might be potential approach of choice in patients that are eligible; otherwise, systemic therapies adopted from the therapeutic armamentarium for multiple myeloma are alternatives [2]. 🔎 When a patient with #adrenoleukodystrophy is diagnosed with a progressive and inflammatory evolution of #demyelination. Patient’s life is at stake: death could occur within 2 to 4 years. There is no treatment available. The current standard of care is a hematopoietic stem cell transplantation (HSCT) to halt the #neuroinflammation. Yet, its efficacy is strongly related to the accuracy and timing of #diagnosis. Too often, patients are diagnosed at an advanced stage and are thus not eligible anymore for HSCT. In addition, HSCT does not seem to prevent the progression of degenerative myelopathy. 🧠 Therefore, there is a crucial need for relevant, sensitive and specific #biomarkers to monitor disease evolution, optimize #care with the right treatment at the right time, and support new disease-modifying therapies development for tomorrow.
