Thyroid Stimulating Hormone (TSH) : Start at the Beginning 1. First Test: The TSH test should be the initial thyroid test for patients with symptoms of thyroid disease. 2. High TSH: Follow up with a Free T4 test to confirm hypothyroidism. 3. Low TSH: Follow up with Free T4 and Free T3 tests to confirm hyperthyroidism. 4. Additional Tests: Consider thyroid antibodies, ultrasound, or radioactive iodine uptake based on clinical presentation and initial test results. Starting with the TSH test allows clinicians to efficiently evaluate thyroid function and determine the appropriate next steps for diagnosis and treatment.
