Introducing our latest report, ‘Data for people’ — a report co-produced with people who draw on care and support, as well as family and friend carers. This new report sets out 15 key principles and 6 actions to improve how care data is collected, stored, shared, and used in England. At its heart, the report reflects the experiences and priorities of people with lived experience. It highlights the importance of equity, inclusion, trust, and transparency, calling for ongoing co-production so that data practices support people to live the life they want. Our hope? That this report informs the government's approach and sparks meaningful improvements across care and support. This is just the start. Let’s keep the conversation going — we'd love to hear what you think about data in care and support in the comments. 🔗 https://bit.ly/3Y9AefE

Join in on a very important conversation! #integratedcare as a concept, has yet a lot of untapped #opportunities. It moves beyond the "classical" way of delivering health & care, to a truly "#integrated" lens that simply connects the dots. The International Foundation for Integrated Care (IFIC) has great resources on this topic. Share your insights on the #survey to contribute to this movement. #personcentredcare #IFIC #Shareddecisionmaking

Many of our improvement efforts in health & care address ‘systemic’ issues: deeply embedded challenges that are complex & difficult to resolve. We can increase the possibility of better outcomes by involving multiple people, using a process framework for systemic change, such as the "Systemic Design Framework" from the Design Council. Some resources that explain the framework: https://t.co/mfSmtDjixj A case study of its application in a community initiative in health & care: https://lnkd.in/eBjBV3xD
