Partners in Care Foundation’s Post

Partners' Enhanced Care Management (ECM) team came together to organize and distribute essential items to our ECM participants. These items are a vital support for individuals and their families, helping to ease the burdens that come with managing chronic conditions. Many of these participants are unhoused, live with severe mental illness, substance use disorders, and other complex health conditions. By providing access to these items, we not only improve physical well-being but also boost mood, bring hope, and recognize the daily efforts of our participants as they work towards their health goals with their care coordinators. This distribution will reach participants across various communities in LA County, Santa Barbara, Riverside County, San Bernardino County, and San Diego. Items distributed include blankets, diapers, body wipes, masks, shoes, and lots more! A special thank you to Shelter Partnership's S. Mark Taper Foundation, a nonprofit organization that shares our commitment to serving high-needs and vulnerable populations, for generously donating 7 pallets, equivalent of $31,000 worth, of essential items.
