Upon meeting Ray, I asked him to inventory his medications. When We noticed Ray was juggling 15 medications, We knew immediate action was necessary. We accompanied him to his appointment because he was confused, sick, and ready to give up. After leaving the appointment, his doctor reduced his medications from 15 to 5 pills. Ray was being seen by multiple providers (systems were not talking). Perhaps in approximately 10-12 minute appointments, doctors were not questioning, or he could not understand medical jargon.
