Did you know Community Health Centers (CHCs) serve over 1.3 million patients who experience homelessness every year? Those facing homelessness are at heightened risk for chronic and acute diseases, behavioral health conditions, and more, which can lead to poor health outcomes, disabilities, and premature death. Our dedicated member CHCs, like Colorado Coalition for the Homeless, are at the forefront, providing essential care, support, and dignity to those in need.
