The latest calculator we developed is based on the #Pediatric #Crohn’s Disease Activity Index (PCDAI) that stratifies disease severity and helps track progress and response to therapy in patients aged 19 and under. The index is based on three groups of findings, patient history information, examination findings and laboratory. The PCDAI maximum score is 100, where the higher the score, the greater the disease severity, whilst each decrease in score by 12.5 points indicates clinically significant response to therapy. The main advantages of the PCDAI compared to the HBI are that it includes growth parameters and objective markers of inflammation, and it offers a well-defined scoring range. PCDAI performed better at discriminating levels of disease activity, compared against the adult Crohn’s Disease Activity Index (CDAI). https://lnkd.in/d6u52QTF

Herpes Zoster Ophthalmicus (HZO): A Concern for Children I recently encountered a 12-year-old patient with Herpes Zoster Ophthalmicus (HZO), a condition that often affects adults. HZO is a reactivation of the varicella-zoster virus, the same virus that causes chickenpox. After chickenpox, the virus remains dormant in the body's nerve roots. Years later, it can reactivate, causing HZO. Causes and Progression: Stress, illness, or weakened immune system can trigger HZO. The virus travels along the facial nerve, affecting the eye and surrounding skin. Symptoms: HZO initially presents with a painful, blistering rash on one side of the face. The rash often follows a nerve pathway. Other symptoms include: * Severe eye pain * Sensitivity to light * Blurred or double vision * Redness and swelling of the eye * Small, fluid-filled blisters on the cornea Prognosis: While most cases of HZO resolve on their own, complications can occur, especially in children. These include: * Permanent vision loss * Corneal scarring * Chronic eye pain Early diagnosis and treatment are crucial to prevent complications. If you suspect HZO, consult a doctor immediately. 💭💭💭 When we studied meningitis, the most common causative agents identified were Group B Streptococcus, Haemophilus influenzae, and Streptococcus pneumoniae. However, in recent cases of meningitis, particularly in rural areas of Sindh (Jacobabad), Klebsiella pneumoniae has been consistently isolated as the predominant pathogen. Klebsiella is a Gram-negative rod, and meningitis caused by this pathogen tends to be more severe and associated with higher morbidity. The increasing cases of meningitis caused by Klebsiella pneumoniae are a serious cause for concern as they usually do not respond to the antibiotics commonly use to treat meningitis. Clinically typical erythemato-squamous papules and plaques of different sizes and shapes on the skin are characterized by the parallel appearance of epidermal hyperproliferation, inflammation and angioneogenesis. Although many factors are blamed in the etiology of psoriasis, genetic and environmental factors play a role in the pathogenesis of the disease. However, there may be fluctuations in psoriasis activity by hormonal changes and pregnancy. Overall, three out of every four psoriasis patients experience a change in the course of the disease during pregnancy. The proportion of those with improvement in the course of psoriasis is about twice as high as those with worsening. However, generalized pustular psoriasis of pregnancy seen in the last trimester of pregnancy can lead to serious maternal and neonatal morbidities and even mortality. On the other hand, some complications such as low birth weight, preterm birth, gestational diabetes, gestational hypertension, preeclampsia and emergency cesarean section have been reported more frequently in pregnant patients with severe psoriasis. Therefore, considering the adverse maternal and fetal outcomes of severe psoriasis and the fetal side effects of systemic psoriasis treatments, psoriasis in pregnant cases should be managed well by keeping the risk-benefit ratio in balance. It is characterized by the sclerosis of portal areas without cirrhosis and with permeable portal and hepatic vein.... #Case Presentation A 4-year-old male child was admitted due to abdominal distension and upper gastrointestinal bleeding. The patient had no significant personal or family medical history, and consanguinity was absent. Physical examination revealed appropriate growth for age, an enlarged spleen (8.5 cm), and a liver size of 18 cm. Collateral circulation was observed on the abdominal skin, and no associated skin manifestations were noted... #Conclusions Hepatoportal sclerosis is a rare etiology of portal hypertension in childhood, it is primarily observed in adults, represents a rare etiology of portal hypertension in childhood.
