When people hear about Value-Based Health Care #VBHC, they often think of measuring outcomes through #PROMs (Patient-Reported Outcomes Measures). But that in itself is NOT SUFFICIENT for creating value from the perspective of patients, and thus, #value for all stakeholders. Integrating care and measuring costs over the whole care pathway are also neccessary. The cost element has garnered lots of attention, but not so much with the integration and the organization of care based on medical condition and patient segments through the use of #IPUs (Integrated Pratice Units). The implmentation of IPUs is only in its early days in North America, but our colleagues in the Netherlands have been applying and improving the model for years. This fantastic article in NEJM Catalyst from one of my favorite thought-leaders and collaborators in VBHC Implementation, Paul Bastiaan van der Nat and collegues, really sums it up well for those looking to optimize health outcomes and costs by organizing IPUs around a medical condition.

Reflecting more on the Canadian Patient Safety Week theme of "what comes to mind when you think of healthcare harm?” - We know that healthcare harm can take many forms. Too often our healthcare system doesn’t see the barriers faced by our most vulnerable people when they access care. Unless products and services are designed with a deep understanding of the complex social, cultural, and emotional needs of the people who will use them, we risk excluding these people from a system that should be helping them. Our design process always seeks to understand and design for the unique needs of all people. Following Wes Streeting's recent announcement at the Labour Party Conference and the insights from the Darzi report, it's clear that a shift from hospital-based care to community and primary care is both urgent and necessary for the sustainability of the NHS.  However, as Dr. Joe McManners aptly highlights, achieving this will require much more than a sudden influx of funding.   At The Access Group, we are keen to play a pivotal role in this transformation. Our systems are designed to support community and mental health services, and we are actively working to digitise patient care through tools such as environment and patient monitoring.  By equipping community services with cutting-edge digital tools, we aim to address some of the capacity and capability gaps.
