One of the things that brings me joy everyday is being closest to those who provide care to our patients, families and the communities we serve. Today I had the pleasure of rounding on the nursing units at Corewell Health William Beaumont University Hospital with Patricia Laurie, Director of Nursing. It was obvious to me that Trish is an engaged leader who is well respected by her team as the interactions seemed very genuine and authentic. ANSWER: Per AANLCP – The steps of the Nurse Life Care Planning process are the same as the nursing process - assessment, nursing diagnosis, outcome identification, planning, implementation, and evaluation. Let’s just be honest, nursing culture has changed. In today’s hospitals and clinics, homes and skilled nursing facilities, rehabs and hospice, whatever the location the challenges are very similar. Nurses feel unappreciated, overworked and are in survival mode, as they take on not only the nursing roles but those of the tech, the unit secretary, the phlebotomist and the RT, sometimes even housekeeping… I see you! But I’m reminded of something I learned in nursing school that has never failed me, the same something I saw a physician do many years ago that earned him so much respect in my eyes, when I asked him a question he didn’t know the answer to. He stepped out of the room and came back in with a big book and found the answer, right in front of me. This was before I was a nurse, I was just a young mom with a sick baby. You see what I learned in nursing school, what that physician showed me, we don’t always have to have a ready answer, we just have to know where to find answers. Though nursing in and of itself is shifting, we don’t have to be afraid of change, we just have to learn how to adapt and inspire those around us. Whether leading your coworkers laterally or in a position of leadership we all have an opportunity to impact the culture of nursing. This book is a great place to start. I just started it a few days ago and already have gleaned a wealth of insight. Tip for Hospice Nurses: How to Document to the Decline, or Negative Documentation. The principle is easy to understand, but not so easy to put into practice. Throughout nursing school we were taught to document positive outcomes, and that is what we did our whole nursing career … until we came to hospice. Now suddenly we have to switch to negative documentation. It’s hard! I struggled with this for years! One thing that helps me is changing the first word from positive to negative. Instead of saying the patient is oriented x 1, I say the patient is disoriented x 3. Say this to yourself and think about the difference in the image the two descriptions produce. Do this with each system - change the first word to the negative description. If you are struggling with this, you are not alone. It does take practice … a lot of practice. I have a couple of books that will illustrate how to do this and give you a whole lot of words and phrases you can use. Tip for Hospice Nurses: In every hospice nurse's career, there will come a time when a Pt or Cg complains and demands a new nurse. 😔 When this happens to me, I feel a mix of emotions - upset, confused, sad, and embarrassed. But deep down, I know I provided the best care possible. 💔 So, in those moments, I remind myself: 1. It's not personal. 🙅♀️ 2. It doesn't reflect my nursing skills. 🩺 3. The Pt or Cg is struggling with anger, fear, and loss of control. This may be their way of trying to regain some control. 4. This is something every nurse experiences. I understand it's easier said than done, but I try not to dwell on it. I share my feelings with friends and peers, openly discussing it during IDT. I've found that when I vocalize what's bothering or embarrassing me, it gradually loses its power until the negative feelings fade away. 🗣️💬 And always remember, there are people who love and support you unconditionally.
