#KaviReporting | The idea of standardizing medical rates presents both opportunities and challenges for hospitals. Standardization has the potential to simplify billing procedures and save administrative overhead by streamlining price structures. Hospitals may thus experience cost savings and operational efficiencies as a result, especially those that are juggling complicated charge schedules. However, they may face difficulties implementing consistent prices, nevertheless, especially in Tier cities where operating expenses differ greatly. Larger patient numbers and economies of scale may make it simpler for hospitals in urban areas to absorb the effects of standardization. Hospitals in Tier II and Tier III cities, on the other hand, would find it more difficult to adapt to uniform pricing, which could put a strain on their finances and cause operational problems. According to Harvard Business Review, simply trimming spend in areas of staff, equipment, and supplies can hinder a hospital's ability to deliver an excellent patient experience. The balance between cost and delivering positive patient experiences is fragile. Last week, I had the pleasure of visiting my #hometownpharmacy after almost a decade. It was impressive to see the advancements in the pharmacy space since that time. This transformation has been driven by digitization, increased patient tools and focused efforts on workflow optimization; resulting in enhanced efficiency in the pharmacy, significant improvements to overall business operations and improved patient experience. In other words, the effort and time spent on optimization has led to a reduction in administrative burden for the pharmacy team, freeing up more time for patient care delivery. This transformation from simple adoption of digital systems and workflows to fullscale optimization is imperative across all areas of the health care system – moving from technology as an information management tool to one that can materially enhance the delivery of care and patient experience. The integration of new digital health tools such as ePrescribe and expanded systems integrations have enabled the pharmacy to offer a broader range of services than ever before. One particularly intriguing development is the expanded role of pharmacists in Ontario, now empowered to prescribe for minor ailments for patients with a history of such issues. This evolution underscores the growing importance of pharmacists in the continuum of care. During my visit, I had the privilege of being guided by Irshad haq the store Associate, whom I worked with in many years gone by. We had the opportunity to reflect on the changes and share nostalgic moments from the past. It's heartening to see that amidst these changes, the warmth and dedication of the people in this space has remain unchanged. While much progress has been made, I'm eager to see the #FutureofPharmacy, it’s evolution and contribution to the broader health care ecosystem. There are many opportunities that lay ahead to further enhance the patient journey and support holistic healthcare delivery. Wellness Wednesday – Does Practice Size Matter? Medicine has seen a progressive shift in physician employment status over the past 15 years, with fewer physicians in independent practice (<50%) and approximately 50% in “smaller practices” per the AMA. The “corporatization” of medicine has led to this change with hospitals, investors and contracting realities driving the aggregation of practices. My favorite organizational model is a core employed medical group surrounded by a “wrap-around” IPA. This way you combine the capital-intensive, but more standardized group model with the more variable, capital-light, small independent practice model. But which providers perform best? Investors are attracted to larger groups due to perceived ease of acquisition and control, and, presumably, profits. But experience has shown that this path is often not predictable for a myriad of reasons. Furthermore, in my experience, it is often long-established solo and small group physicians who have a complete understanding of their practice, patients, and community, as well as exceptional motivation, that often perform best in value-based care. So, although no practice model guarantees success in value-based care, solo and small practices are a valuable, but often ignored, part of the solution. Blind spots in healthcare: how fragmented patient views are putting patients at risk 🩺🔍 As caregivers, we often find ourselves piecing together a patient's story, like a puzzle with missing pieces. It's not easy to make a truly informed decision! Consider the challenges faced by retail pharmacists worldwide. They dispense medications without always knowing the full context of a patient's history, especially pathologies or biological vitals. This lack of a complete picture can lead to serious adverse drug events and suboptimal care. Doctors face similar hurdles. A primary care physician might prescribe a medication, unaware that a specialist has already done so. These disconnects can have grave consequences for patient safety. Initiatives like Mon Espace Santé in France and Surescripts in the US aim to centralize and share patient data, but there's still work to be done. Real-time insights into patient compliance and treatment understanding are often lacking. That's why initiatives such as "@Healthshare" from InterSystems, which enables data sharing between health systems, are so valuable. 📊 To truly bridge these gaps, we need to foster greater coordination among all caregivers. Pharmacists, in particular, play a crucial role in connecting the dots between hospital and home, thanks to medication reviews that can be shared with all healthcare professionals. Is there hope for more effective medical care yet lower healthcare costs? Point-of-care testing (POCT) is a promising technology which provides a type of monitoring that enables more frequent measurements, shortens response time, and reduces the frequency of doctor’s visits. Part of this new system are process digitalisation and the integration of services as well as results into a single platform. In medicine, the term ‘point-of-care testing’, also known as rapid testing, refers to diagnostic testing that is not performed in central laboratories, but in pharmacies, doctors’ surgeries, ambulances, hospitals or the recipient’s home, the results of which are obtained on the spot without the involvement of a laboratory. POCT will facilitate the transformation from a static healthcare system into a dynamic one: In the future, the clinical workflow will become broader, covering not just conventional face-to-face consultations, but also self-testing at home, as well as phone and video consultations. Digitalisation is leading to more effective medical care and should help to lower costs. In addition, the patients’ quality of life is improving, as they can stay at home longer when they are ill and have to make fewer physical visits to their doctor’s practice. The COVID pandemic has shown that a significant portion of the Swiss population is sceptical about technical progress in medicine. In this context, it is essential to resolve issues around data ownership, data protection and data sovereignty.
