A local trade union leader "linked the incident to the use of private sub-contractors at the facility". He may well be right, but I'd imagine they're simply the unlucky souls who were at the sharp end of a collection of other issues. Contractors are an essential part of may operations and can bring a wealth of expert knowledge that we can't generate internally in the broader organisation. But there are also control-of-risk/work issues as well since they are not a normal part of our team. I can almost hear the discussions that are no doubt going on right now: "but they all did the induction, and signed a piece of paper to say they'd follow our rules. How could this happen?" Almost as sad as the three people losing their lives is the fact that this is happening everywhere, every day. Management systems have to deal with contractors in a different way to full time team members, which isn't just 'sign here, follow our golden rules or you're off the site'. I hate speculating about stories in the rags as the amount of information is minimal. But if it walks like a duck..... #safetyculture #safety #hse #incidentinvestigation
