Autoimmune disease is a logical extension for chimeric antigen receptor T cell (CAR-T) therapy. Historically developed for cancer treatment, CAR-T cell therapy has recently demonstrated remarkable success in treating autoimmune diseases. In this study, as in cancer treatment, CAR-T cells target a specific cell type, such as CD19 in B cell malignancies and B cell-driven autoimmunity, as reported at the American Society of Hematology (ASH) meeting in San Diego, CA. https://lnkd.in/g5cQ_PhD However, the therapy for autoimmune diseases involves intensive chemotherapy to eliminate existing immune cells; yet, significant success was achieved even with the risks of immune ablation therapy in patients with autoimmune diseases. This breakthrough offers new hope for conditions like lupus, systemic sclerosis, and idiopathic inflammatory myositis. To mitigate these risks, one could envision developing a CAR-T therapy where the chimeric antigen receptors target a specific cellular subset, such as only those B cells with the disease-driving paratope. This approach could potentially reduce the need for immune ablation therapy. We are in an exciting era where cells are becoming therapeutic agents.

Opioids, while effective, cause dosing issues due to side effects and addiction concerns. Some patients don't respond well to opioids. New non-opioid treatments are needed. For CRS, management escalates from supportive care to tocilizumab and finally corticosteroids for more severe or prolonged cases. ICANS is treated with prophylactic anti-epileptics if not started previously and corticosteroids. Studies are also optimizing prevention strategies, like earlier intervention thresholds for tocilizumab/steroids without efficacy impacts. One ASH 2022 abstract that Dr. Leslie’s center participated in utilized prophylactic anakinra (an IL-1 receptor antagonist) in lymphoma patients receiving commercial CAR T products to reduce severe neurotoxicity rates. Only 10% experienced severe ICANS, all grade 3, and 30% had any grade ICANS overall, favorable compared to historical benchmarks. Thus, preventative measures like anakinra enable safer CAR T delivery in more patients.

Several mechanisms can lead to the link between chronic PPIs use and "gastric cancer". Past studies on rodents showed that potent inhibition of gastric acid secretions had induced gastric cancer, associated with secondary hypergastrinemia that resulted in enterochromaffin-like cell hyperplasia (a carcinogenic effect). Although PPIs remain the best agent to control gastric acid secretion and are useful in many indications, especially those related to the gastrointestinal tract (such as peptic ulcers and H. pylori), their chronic use must be observed.
