Versatility in Case Review I recently had a client reach out and ask me to assist with a case. He said he knew that the case involved an area I didn’t necessarily have experience in at the bedside. However, he wanted me to review the records because he trusted me and my work product. I was asked to complete a condensed chronology that included my analysis/comments (for him) but also one that didn’t so that it could be sent to an expert for review with the records. Behind-the-scenes LNC work is unique. Depending on what is being asked of you, you don’t necessarily have to have the exact experience in the particular area the case and claims are about. For example, most of my nursing experience is inpatient medical-surgical, telemetry/cardiac, and spine/ortho surgery. However, I’ve assisted with cases that have involved injuries sustained in the OR, ICU, ED, outpatient doctor’s office, etc. With my experience as a bedside nurse, I: ➡ Can extract the relevant information based on the claims and injuries. ➡ Have developed the necessary critical thinking skills to analyze the records. ➡ Can research standards of care in a particular area or reach out to those with experience in that area if unsure. ➡ Am able to research different medical aspects I may not be 100% familiar with so that I can educate my attorney-client and give them nursing insights into the records. I will be the first to pass on a case (or sub it out!) if I am not 100% confident I can produce high-quality and accurate work. Like OB/birth injury cases…..that’s a no from me. I wouldn’t even know where to begin! As a behind-the-scenes LNC, I am very versatile in the types of cases I can review!! During the filming of Scattered Denial, we met a few physicians who wanted to speak out on the dangers of radiation and the scourge of orthopedic injuries in interventional labs, but felt that they couldn’t do so. In some instances, the simple fear of rocking the boat with their hospital administration was enough to dissuade them from participation. In other cases, hospital administrations issued outright vetoes and implicit ultimatums. We were disappointed to hear that some hospital administrators were not only reluctant to invest in devices to protect their staff, but were also preventing their employees from sharing their experiences—largely experiences dealing with physical pain endured in order to do the job they love and have trained countless hours to perform. As new technologies are introduced yearly, there are many legitimate conversations that will need to be had between providers and hospital administrations. And in smaller systems or upstart OBLs, there will likely be unavoidable financial tradeoffs. Perhaps instead of investing in a comprehensive, group protection shielding mechanisms for all their labs, some hospitals may find it more appropriate to invest in lightweight vests as a start. Sometimes, it may be necessary for administrations to make such decisions after listening to physicians, nurses, and techs, and fairly weighing workplace safety concerns with financial realities. However, it is our belief that this problem will not be solved without a public dialogue taking place, and that enforced silence is a shortsighted path leading to poor outcomes for physicians, patients, and the hospitals themselves. If some hospital administrators deem adequate protection too costly, there are certain questions they would be wise to ask themselves. Questions like: - What would the financial implications be of losing our top operator to a career-ending back injury? - If we are already understaffed, how will we handle losing techs and nurses who are entering the prime of their career? - Will our hospital be an attractive landing spot for new graduates if we don’t invest and lead the way in safety? - Is it right that our employees may experience avoidable disabilities as a result of working for us? In the words of the esteemed Dr. William O’Neill, “An interventional cardiologist is willing to take the risk, because they want to do the greater good. And I think that’s incredibly admirable . . . . I am a pharmacist by background, my initial years of career as a pharmacist were in burns and trauma ICU, ACU, stepdown unit, medical surgical unit, orthopedic surgery, anticoagulation clinic, drug information, inpatient pharmacy, ambulatory pharmacy, discharge pharmacy, and briefly NICU, and delivery and maternity wards. I have been privileged with work and exposure to various areas of clinical care. As I transitioned into quality and medication safety, and now the wider scope of patient safety, I have developed a comprehensive view of the flow of healthcare in different settings, with understanding of risks and bottlenecks in the processes. I have also been privileged with a network of mentors in formal and informal capacities who still are my safety net and a rich resource of support and knowledge. I am privileged with my life partner who shares the same mission, vision, and values of delivering excellence. We have engraved into the belief system of ours and our children's that investing in ourselves, our personal and professional development, is our top priority. Having known and practiced handson excellence in healthcare, in every step, every decision, and every experience, I know that I am delivering the best of my knowledge and skills, every single day. I have learned to find the learning in every experience; every mistake, every feedback in any form, every challenge, and every celebration. I am not listing my CV, nor trying to impress, but merely thinking and reflecting out loud on conversations and experiences I have had over the past few weeks that further anchored my direction and choice of my career path. It is not simple nor easy to work in the field of quality and patient safety. It is time to change the common misconception that it is something anyone can do without prerequisite experience or qualifications. It is indeed everyone's responsibility, but it takes a lot more than interest and years of existence in an organization to become a quality and patient safety professional.
