Here is a story from Ken , a psychiatric nurse at St. Patricks hospital and our mobile clinic: Life-Changing Diagnosis: How a Kids' Screening Program Helped a Little Girl with Inguinal Hernia In our ongoing efforts to enhance community health, our Kids' Screening Program has proven to be a beacon of hope for many families. Among the many success stories, one particularly heartwarming case stands out: the diagnosis of a 4-year-old girl with an inguinal hernia, a condition that had previously gone undetected. During our Kids screening sessions at Amaning, a local primary school, young Zakiya was brought in. Her mother, Ayishatu, had noticed she often complained of discomfort and occasional swelling in her groin area but had not been able to pinpoint the cause. They hoped the comprehensive check-up provided by our program might shed some light on their daughter's condition. During the screening, our healthcare professionals conducted a thorough examination of Zakiya. It was during this exam that our pediatric specialist identified signs of an inguinal hernia—a condition where a part of the intestine protrudes through a weak spot in the abdominal muscles, often leading to pain and swelling. This diagnosis was crucial, as inguinal hernias can lead to serious complications if left untreated. Understanding the urgency of the situation, our team immediately scheduled Zakiya for a follow-up consultation with a pediatric surgeon. Recognizing the financial strain this could place on her family, we assured Ayishatu that the surgery would be provided free of charge as part of our outreach program's commitment to the community. The Mother's Gratitude: Ayishatus overwhelmed with relief and gratitude. "I can't express how thankful I am for this program, I had no idea what was causing Zakiyas discomfort, and now, not only do I have an answer, but she's also getting the help she needs without the financial burden." The American Academy of Pediatrics needs your help! They are looking for pediatric providers to complete a survey to help them better understand children’s #mentalhealth care in #pediatric #primarycare settings. AAP is gathering insights related to current mental health screening practices, resources used, and barriers or challenges faced; mental and behavioral health training needs; Pediatric Mental Health Care Access (PMHCA) program awareness, usage patterns, and barriers or challenges faced; and primary healthcare training needs. The AAP will use this information to develop resources and implement technical assistance activities to support pediatric providers in promoting and addressing mental and relational health with children, adolescents, and families. Completion of this survey is completely voluntary. It should only take 10-15 minutes to complete. Survey participants may refuse to answer any question for any reason, and they may stop at any time. Artificial Intelligence AI access is the new thing. I remember last year when some of the older school children were talking about how it has helped speed up research I was a bit surprised at how the schools were allowing it but quickly realized that quick access to information is what the younger generation is used to and with some rules this can be a game changer. We just need to make sure we emphasize and teach our children how to use it correctly. I think as new things come out we all should embrace advancements and use them appropriately. I use AI as a note writer and its amazing because it allows me to just talk to the families without having to type. The Struggle to Prioritize Prevention and Early Diagnosis in Pediatrics Working in a pediatric office can be extremely rewarding but also challenging when it comes to preventive care. As clinicians and #pediatricians, we understand the importance of preventive services like screening, counseling, and early intervention. However, implementing these services effectively often takes a backseat due to significant barriers: 1) Time Crunch: The realities of running a busy practice leave little room to devote sufficient time to preventive care. Between addressing acute issues, managing a full patient load, and handling administrative tasks, there's rarely enough bandwidth for in-depth preventive counseling and screening. 2) Financial Constraints: Preventive services are often inadequately reimbursed, making it difficult for practices to allocate resources and staff towards implementation. The focus inevitably shifts to revenue-generating services that keep the lights on. 3) Knowledge Gap: Staying up-to-date on the latest preventive guidelines and evidence-based approaches requires ongoing education and training, which can be challenging amidst ever-increasing clinical demands. 4) Workforce Shortages: The shortage of pediatricians, pediatric sub-specialists, and support staff exacerbates the time crunch, leaving even less capacity for preventive and early diagnosis care initiatives. 5) Difficult Conversations: Providers may experience anxiety about delivering potentially upsetting news, such as discussing mental health concerns, early diagnosis of disease without a cure, or unhealthy behaviors with families. While these barriers may seem daunting, they are not insurmountable. To better support preventive care in pediatrics, we need systemic changes. This could include improved reimbursement models that value preventive care, better integration of preventive services into workflows, increased training opportunities to stay updated on the latest guidelines, and addressing workforce gaps. A multi-pronged approach is required, and each of us has a role to play in this. Prevention is key to promoting children's health and well-being. By acknowledging and addressing these barriers head-on, we can work towards a future where preventive care is seamlessly integrated into pediatric practice, leading to healthier outcomes for our youngest patients. We are leaving #children behind when we don't diagnose #autism, #T1D, #suicidality early and intervene to change the natural history of the disease.
