Case Report: Prescribing Based on Presenting Symptoms First Consultation: Presenting Symptoms: - Arthritic pain in the wrist joint. Observations and Analysis: - The patient's symptoms predominantly pointed towards a rheumatic or gouty diathesis. - The pain being localized to the wrist joint was a strong keynote for *Actaea spicata*. - *Actaea spicata* is well-suited for arthritic pains in small joints, particularly in women. Prescription: - Rx:Actaea spicata 30 C, single dose. - Advised to return for follow-up in case of persistent or new symptoms. Outcome: - Significant improvement in wrist joint pain reported by the patient. --- Second Consultation: *Opens up, and wants to discuss more in detail* - Persistent gastric complaints. - Heel pain experienced constantly especially on waking up in the morning. - Brittle, yellowish black, and hard in-grown toenails. - Acidity after drinking lemon and lukewarm water in the morning. Observations and Analysis: - The combination of gastric disturbances, nail changes, and heel pain pointed to a psoric or sycotic miasmatic background. - Rx Antimonium crud. matched the totality of symptoms: - Gastric complaints, particularly aggravation from sour substances like lemon. - In-grown, brittle nails, often discolored or deformed. - Pain in heels, possibly linked to calcaneal spurs or chronic inflammation. Prescription: - Rx: Antimonium crud. 30 C for one week. Reflection: This case highlights the importance of careful observation and remedy selection based on presenting symptoms in acute and follow-up consultations. While the first prescription (Rx. Actaea spicata) was targeted at localized arthritic pain, the second prescription (Rx.
