In 2023, failures in lockout/tagout (LOTO) procedures led to several fatal incidents. OSHA estimates that proper compliance with LOTO standards could prevent around 120 fatalities and 50,000 injuries annually. However, violations of these safety measures continue to be a significant issue. In 2023, there were approximately 2,139 LOTO-related citations, with several tragic fatalities reported as a result of improper energy control measures during maintenance and servicing activities.
These fatalities often involve workers being caught in or crushed by machinery that had not been properly de-energized, as well as failures in safety training and enforcement of LOTO procedures. While specific fatality numbers directly tied to LOTO violations in 2023 are still being compiled, OSHA data from previous years suggests that LOTO failures consistently contribute to a considerable portion of workplace deaths.

“We’ve always done it this way”
These are some of the hardest words for a safety professional to hear. Especially when they come from your son talking about a supervisor on his new job site.
My son recently started his electrical apprenticeship with a well known company in the local area. Yesterday he came home and was talking to me about his day at work. While talking he mentioned that his supervisor on the site was working on something that was still live and said, “you shouldn’t do this but I don’t want to go all the way to the panel.” I was surprised to hear that this happened. I was even more surprised that the supervisor said those words out loud and then did it anyway, in front of two future electricians.
We talked about LOTO and we also talked about speaking up when safety is involved. Being new can make it tough to speak up. So we played a few things out. In this case, we deciddd next time my son will simply say, “can you show me how to shut this off at the panel first?” Or, “I will go check the panel next f you want”. This allows the supervisor to think twice and teach the right way instead of building bad habits.

If what is contained in the article is true, it sounds like the Chicago Transit Authority has some work to do when it comes to employee safety!
The gloves the employee was issued to “protect” them from caustic train cleaning chemicals are clearly lineman’s / electrician’s gloves and are not rated for chemical use!!! In fact, the manufacturer recommends that if they come into contact with any chemicals, that the gloves should be removed from service, immediately cleaned and tested by the manufacturer.
Disciplinary action against employees who refuse to do work because the work is unsafe if about as low as you can get and will result in serious liability for the taxpayers of the City of Chicago.
Employers MUST do better than this. There is zero excuse for allowing unqualified workers to use corrosive chemicals without proper training and PPE.

Just one video this week as the trainee course completed the third week with BA. This week they have mainly been completing BA wears in a variety of situations ahead of assessment next week. However we have spent some time looking at fire behaviour including this session discussing trench effect, and the ferocity of flames when the trench is enclosed.
Understanding how fire behaves is a crucial skill for firefighters allowing them to predict development and select the best approach to extinguishing.
