Between 2022 and 2023, some died from a workplace accident every 96 minutes. From 2021 to 2022, the rate was every 101 minutes. Bureau of Labor Statistics ( U.S. Department of Labor)
The U.S. Chemical Safety and Hazard Investigation Board (CSB) reported that in fiscal year 2023, the number of reportable incidents increased by 51% compared to the previous year. Incidents involving serious injuries rose by 11%, and those involving deaths increased by a notable 78%.
Training alone obviously is not making these issues better. As noted by the CSB investigation of the DeepWater Horizon accident, they quoted the following: Procedures are n=more than a set of instructions; they are tools for competent, motivated individuals to plan, coordinate, verify, and assure performance will achieve the intended results.
