Reflecting on my journey as an ED resident, here are 5 key principles I wish I knew from the start: 1️⃣ Never assume a patient's vitals in the ED. Always recheck before making decisions as a patient's condition can change rapidly. 2️⃣ In emergency medicine, our goal is not just to react to crises but to anticipate and prevent them. Recognizing the signs of potential deterioration in a patient's condition and taking proactive measures can make all the difference. It's about staying vigilant, being prepared, and acting swiftly to ensure the best outcomes for our patients. 3️⃣ Don't hesitate to ask for help when performing a new procedure, regardless of how busy the ED may be. 4️⃣ Maintain professionalism at all times. The ED can be a stressful place and emotions can run high. Avoid arguing or shouting in front of patients as they deserve our utmost respect and care during their difficult times. 5️⃣ Patients arriving after midnight often require admission. Even seemingly trivial complaints warrant thorough evaluation, especially during night shifts when fatigue may impact diagnostic accuracy. Let’s not send them home ! Let's keep these principles in mind as we strive to provide exceptional care in the ED. In collaboration with the Getting It Right First Time (GIRFT)’s Urgent and Emergency Care team, we have published new guidance that outlines core principles to guide actions and behaviours for effective front door acute patient care. Our clinical vice president, Dr John Dean, said: ‘We know that delivering acute care for medical patients needs joint working of specialties. Last weekend, while my wife and I were in Indianapolis, I got to have lunch with my friend and former EMS Medical Director, Mike Russell; During all those years of being his assistant as part of my role as EMS Liaison at the hospital, I got to use my strengths to complement his; my analytics and improvement mindset allowed him to explore topics that he didn't otherwise have time to research (working full time in the ER, going on international missions trips...). He modeled compassion and temperance while we talked through clinical issues with Providers: "We weren't on the call. Tell us about it." were his opening lines--it reinforced the importance of being quick to listen and slow to speak. Our monthly audit & review sessions for our 300+ affiliated EMTs and Paramedics relied upon our teamwork. I got to be at my best, so he could be at his best, to help all of them be at their best. That's a good thing, as he would say. Now, I'm excited to bring this same teamwork to other Medical Directors who realize they would love to do more for their Providers and agencies. Quality, continuous improvement, and strengths put me into my zone of genius--I get to take this work off of their plate so they can give attention to the things they'd rather focus on. Sometimes, people are surprised at what they didn't know they were missing--from detailed cardiac arrest reviews and 12 lead case studies to EMR-based analytics and best practice reviews, it's like having a virtual EMS assistant. For them. For their folks. For their communities. I got into EMS to help people. I didn't figure I'd be helping people these ways. ‘We ensure that medical and allied health decisions happen at the same time. That's the key to speeding up the process.’ Today we're diving into the world of patient discharge with Connor, a #TeamCHS Clinical Care Coordinator. From the moment a decision is made for a patient to leave the hospital, Connor's role comes into play. ‘I touch base with the patient to ensure they understand what's happening. Then, we transfer them to our discharge lounge to start preparing for the next patient. I liaise with the junior medical officer and pharmacist for any medication or paperwork needs.’ But how do they keep the process seamless during busy times, like chaotic Fridays? Connor shares their strategy: ‘We maintain an estimated date of discharge for each patient, updating it regularly. We prioritize discharges in our multidisciplinary team meetings, ensuring we work from discharges down to avoid delays.’ ‘Patients are often ready to leave despite pending tests. They are aware, up, and ready by 10am, fully understanding their journey.’ Connor's role isn't just about discharge; it impacts the entire hospital. ‘It's about getting patients back to their normal lives outside the hospital. If I'm efficient, we discharge patients faster, reducing their length of stay and opening up beds for those who need it most.’ So, what's Connor's advice for the team? ‘Collaborate, explore all options, and think creatively. We all want patients to return home as soon as they can, so let's brainstorm with them and make it happen.’ ‘It's about getting people home with answers, and that's the best part.’ #TeamCHS #SafeDischarge #PatientSupport #HospitalJourney
