We enjoyed collaborating with Elevance Health to contribute to their Advancing Health Together report. Understanding the nuances of Medicaid population management enables healthcare providers to better serve the needs of this community by dismantling barriers to equity and accessibility. Working directly with our patient populations on our Social Determinants of Health initiative at PM Pediatric Care has allowed us to take a closer listen and establish solutions that more directly meet the needs of our community - not just to survive, but to thrive! Today's focus for National Health Center Week is: Healthcare for people experiencing homelessness. Did you know Community Health Centers (CHCs) serve over 1.3 million patients who experience homelessness every year? Those facing homelessness are at heightened risk for chronic and acute diseases, behavioral health conditions, and more, which can lead to poor health outcomes, disabilities, and premature death. Our dedicated member CHCs, like Colorado Coalition for the Homeless, are at the forefront, providing essential care, support, and dignity to those in need. A large majority of maternal death rates are preventable. Access to information supporting healthy births and mapped digital care journeys linking in-person visits to digital experiences should be the norm for expecting mothers from pre-pregnancy through postpartum. It's time we build a more consumer-centric health system that actually makes people better. What a great partnership to tackle a huge issue.

Consumer Directed Service Coordinator | Empowering Independence for Elderly and Disabled Individuals
Urgent attention is needed for policies that address the unprecedented demographic shift in our aging population. First, we must prioritize aging in place support, preserving older adults’ ability to live at home and in their communities. This requires robust in-home care, transportation, respite services, and home-delivered meals. Second, healthcare access remains critical. The Affordable Care Act (ACA) recognizes the value of prevention and expands Medicare coverage for preventive care. Third, families should create emergency care plans in advance, ensuring legal documents (such as advance directives) guide decisions during serious medical emergencies. Lastly, policymakers must plan for the growing demand for long-term care, as nearly 70% of older adults will need an average of three years of such care .

Like many FQHCs, HealthLinc has limited resources—and their community health workers (CHWs) needed a better way to support patients with social needs. So, in partnership with Phreesia, HealthLinc implemented the Patient Activation Measure® (PAM®) to help better understand the kind of support its patients need. Now that PAM is integrated into its workflows, HealthLinc’s CHWs have been able to connect 43% more patients with relevant SDOH resources.
