We enjoyed collaborating with Elevance Health to contribute to their Advancing Health Together report. Understanding the nuances of Medicaid population management enables healthcare providers to better serve the needs of this community by dismantling barriers to equity and accessibility. Working directly with our patient populations on our Social Determinants of Health initiative at PM Pediatric Care has allowed us to take a closer listen and establish solutions that more directly meet the needs of our community - not just to survive, but to thrive!   Read more here: https://lnkd.in/eGGVyY5r  

  • “One of our Maryland Office Managers reached out to me... with a remarkable email - she said she had been talking to parents in the office reception area, trying to understand what resources they really need help with, and suggested we add a few more referral options for parent education and job training programs in the area [using an online social care network]. 
In the first week of launching SDOHPIP, four families signed up for GED prep courses at Prince George's Community College thanks to that office's SDOH referrals. Having a GED greatly expands a parent's employment opportunities and increases their likelihood of earning a higher income to provide for their family's basic needs.”
