The Caliper: A Strabismus Surgeon's Unsung Hero In the meticulous world of strabismus surgery, where precision is paramount, the humble caliper emerges as an unsung hero. Castroviejo caliper (shown in the picture) helps in measuring the amount of muscle recession or resection that needs to be done in mangement of strabismus. It promotes consistency in the surgical procedure and aids the surgeon in achieving a more standardized and predictable outcome. Precise measurements translate into better alignment of the eyes post-surgery, improving both binocular vision, fusion and cosmesis. Proper alignment and tightening of the pedicle screw/rod assembly after instrumented posterior fusion of the lower spine is known to be crucial in order to achieve satisfactory clinical results. Such interfacing angle mismatches indicate stress overloading of the implant system. With an incidence of 42.1%, pedicle screw/rod interfacing angle mismatch is a frequent occurrence after fusion surgery. Mismatch, which is the angular deviation from 90° formed by the rod axis and the pedicle screw head axis, indicates that it was impossible to obtain a secure form-fit between the pedicle screw head and the rod. The use of torque limiters is intended to ensure that the set screws are tightened securely and that the rod is brought into the desired mechanically ideal 90° form-fit position. The objective of this study is to investigate the incidence of postoperative screw/rod interfacing angle mismatch and to analyze the impact of mismatches on clinical outcome in terms of (1) revision surgery, (2) adjacent segment degeneration (ASD), and (3) pain. 🌟 New Publication 🌟 I am thrilled to announce the publication of our latest study titled "Rotational Stability and Refractive Outcomes of a New Hydrophobic Acrylic Toric Intraocular Lens" in the Top-Journal "Eye and Vision". Our study focused on evaluating the performance of the new Clareon toric intraocular lens (Alcon), assessing its rotational stability and the resulting refractive outcomes in patients undergoing cataract surgery. Key highlights include: 🔍 **Rotational Stability**: The Clareon toric IOL demonstrated mean absolute rotation of just 1.33° from end of surgery to 6 months post-op. 📏 **Refractive Precision**: Postoperative astigmatism was significantly reduced, with mean residual refractive astigmatism measuring below 0.50 D at the 6-month follow-up. ICYMI: Cornea Update: An Unexpected Incidence of Ocular Surface Neoplasia on Pterygium Surgery. A Retrospective Clinical and Histopathological Report: Purpose: To report the unexpected finding of ocular surface neoplasia (OSSN) on conjunctival tissue removed during pterygium surgery and subjected to histopathology analysis. Methods: This is an observational, descriptive retrospective study in which all the samples removed during surgery between 1997 and 2020 with a clinical diagnosis of pterygium were sent for histopathology analysis in which an unexpected OSSN was found. Results: There were 461 cases with a preoperative diagnosis of pterygium, with a mean age of 30.14 ± 10.6 years. In this group, there was an unexpected finding of OSSN in 69 cases (14.96%) of the surgical samples, with a mean age of 46 ± 8.2 years (31.86 ± 12.13). Conclusions: The unexpected coincidence of pterygium and OSSN is frequent, although varies depending on the geographical location.
