Thinking about the management of ME / Chronic Fatigue Syndrome 🤔 (as it’s Severe ME Awareness Day) - one GP / Lifestyle Medicine Dr’s perspective 🤓 ME / CFS and the similar but more pain-predominant Fibromyalgia are high prevalence conditions. As a GP I know I certainly see them almost every working day in some form. Sometimes multiple cases per session. These conditions can be devastating for the patient as well as impacting significantly on the people round them too. There is huge range in severity, but also variability of symptoms and severity within each individual. There is unpredictability. The conditions are still not fully understood, which is challenging for patients and clinicians alike. There are effects on energy production in the body, pain sensitivity, dysregulatiom of the immune system, the autonomic nervous system and more. Complicated. They require optimal management. In General Practice we can make the diagnosis and endeavour to manage symptoms the best we can with somewhat limited options available. More severe cases will inevitably see various secondary care specialists too (to rule out other conditions). Specialist ME services will vary from location to location, but seem generally an end point for diagnosis (where there is uncertainty), support and acceptance of the condition. There may be input from mental health practitioners, dieticians, physiotherapists, pain management teams. Education, insight and self management skills are valuable and necessary. Moving forward, considering the complexities, the uncertainties, the impact on patients and families, and let’s include the economic impact too, what about a deeper dive? 🤔. What about diving into potential contributing individual risk factors and then personalising management for maximum benefit? For example; — Personalised Medicine to offer possible insights into individual risk factors for dysfunctioning systems (biochemical, immunological and otherwise) and therefore best routes to optimal management of symptoms 🧬 —individualised Lifestyle Medicine strategies supported by appropriately intensive high quality health coaching - to optimise overall health and well-being, manage symptoms and address individual modifiable risk factors 💖 —Integrative Medicine (combine conventional and complementary modalities) - such as include the expertise of highly knowledgeable Nutritional Therapists to assess and optimise nutritional status 🍊 (especially important when considering energy production, cellular function etc). Aside from medications, the rest of management (currently) is essentially lifestyle in its various forms.
