Ali Husain Lindsey Maclay, alongside her mentor Dr. Syed Ali Husain, uncovers disparities in access to timely waitlisting among pediatric kidney transplant candidates. Their research revealed significant differences in preemptive waitlisting among pediatric patients, particularly among female, Hispanic, Black, and publicly insured patients. They also identified differences in access to pediatric prioritization among these same populations. Lindsey's work highlights the need for policy revision to address disparate access to pediatric priority in kidney allocation and ensure all children have equitable access to life-saving kidney transplants. 🚨 New Study Assessing Epidural Analgesia in Pediatric Transplant Patients! 🚨 🤔 Are you curious how epidural analgesia impacts recovery in children undergoing heart and lung transplants? This study published in RAPM explored this question with some interesting findings! ✨ Study Highlights: 62 pediatric heart/lung transplant recipients were evaluated by epidural status and assessed for length of hospital stay in addition to morbidity and mortality measures. ✨ Key Findings: - Epidural analgesia reduced hospital length from 26.5 to 20 days in univariate analysis. However, there was no significant difference in length of stay after adjusting for confounders. - Univariate analysis demonstrated a reduced 5-day postoperative opioid requirement and reduced time on postoperative ventilation (7 to 2 days). - Notably, epidural analgesia was not associated with a change in overall survival. ✨ Take Away Points: While epidurals may enhance recovery for pediatric transplant patients, more data and larger studies are needed to confirm these trends and fully understand the benefits. Ophthopedia Update:Long-term outcomes of corneal crosslinking: Purpose of review This manuscript summarizes contemporary research from 2018 to 2023 evaluating long-term (≥2 years) outcomes of corneal crosslinking (CXL) for progressive keratoconus (KCN). Recent findings The standard Dresden protocol (SDP) has been utilized clinically since the early 2000 s to treat ectatic disorders, primarily progressive KCN and postrefractive ectasia. Various modifications have since been introduced including accelerated and transepithelial protocols, which are aimed at improving outcomes or reducing complications. This review summarizes data demonstrating that the SDP halts disease progression and improves various visual and topographic indices (UDVA, CDVA, Kmax, K1, K2) up to 13 years postoperatively. Accelerated and transepithelial protocols have been found to be well tolerated alternatives to SDP with similar efficacy profiles. Studies focusing on pediatric populations identified overall higher progression rates after CXL. All protocols reviewed had excellent safety outcomes in adults and children. Summary Recent studies revealed that SDP successfully stabilizes KCN long term, and a variety of newer protocols are also effective. Pediatric patients may exhibit higher progression rates after CXL. Further research is required to enhance the efficacy and ease of these protocols. #Ophthalmology #Eye #Ophthotwitter

📃Scientific paper: Single step track dilatation for percutaneous nephrolithotomy in children Abstract: Introduction and objectives Data on the use of single step dilatation technique during pediatric percutaneous nephrolithotomy (PCNL) in the literature is sparse. In this prospective randomized study, we aimed to compare the safety, efficacy, and perioperative complications of single step versus serial tract dilatation using Alken metal telescopic dilators during pediatric PCNL. Methods Patients undergoing PCNL were randomized into two groups according to the dilatation technique used. In group A, Alken telescopic serial metal dilatation was utilized, and in group B, single step dilatation was performed. Inclusion criteria included children < 18 years with stone burden from 2 to 4 cm, located in the renal pelvis ± one calyx, who were candidates for PCNL. The primary outcomes were access time and complications’ rate. The secondary outcomes were dilatation fluoroscopy time, operative duration, stone free rate, postoperative hospital stay, hemoglobin deficit, and need for blood transfusion. Both outcomes were evaluated and compared between both treatment groups. Results A total of 70 patients were randomized into group A (35 patients) and group B (35 patients). Access was successfully obtained in all procedures. All the procedures were performed through a single tract. Access time and dilatation fluoroscopy time were shorter in group B (statistically significant). Patients in group A had higher rate of complications (statistically significant).
