"_we just need to join up the data_" - in over a decade of work to improve safety for people most likely to experience harm, I have so often heard this phrase. Sometimes after it's too late. But for so many structural reasons - sometimes good ones, sometimes less so - this is not as easy as it might sound. This proposal to link data of #children and #youngpeople presenting at A&E will be an important step in showing how to ethically join data in order to prevent further harm. New report published by @TheKingsFund today focusing on why the health and care system in England must shift its focus away from hospital care to primary and community services if it is to be effective and sustainable. In response to the report, Anna Hemmings, Joint Chief Executive of the Association of Directors of Adult Social Services (ADASS) said: "The evidence in this report is clear, now is the time to act. Last April we published a roadmap to transform social care in England. Shifting resources into preventing ill health instead of letting people get to crisis point was central to that plan. But it will only work if the same shift happens in NHS care too, as this report argues. We need to make the shift to preventing ill health the political focus of the next decade to make sure everyone can get the care and support they need, when they need it.” Read the report: https://lnkd.in/ehsGk8Nr

New report this week from The Kings Fund highlighting all the challenges to shifting care left, out of hospital and towards primary, community and prevention. So many things have to happen to enable this, it seems overwhelming. However, right at the heart of the report data gets a mention. Whilst the data we work with is never perfect, linking it all up for systems, adding in data on the wider determinants of health and presenting it in the right way is the start of changing the conversation. It's the start of enabling teams to work differently. It's the beginning of thinking differently and planning for about future population and workforce needs. It's a new perspective and a move to view population, people and needs rather than organisations, diseases and activity. It's a way to look back and recognise the progress teams are making towards outcomes shared across system providers and local communities. Whilst the data is never perfect, without joining it up, it's hard to get started. I work in a team who are passionate about linking data, keeping it simple and using it to drive changes like those described in this report. Did you catch the WKYC news segment featuring a community health worker (CHW), Fonda McLaine, and her client, Nadine Head, discussing "Addressing Cardiometabolic Health Inequities by Early Prevention in the Great Lakes Region" (ACHIEVE GreatER)? This research project, funded by the National Institutes of Health, is a multi-stakeholder partnership among Better Health Partnership, Case Western Reserve University, University Hospitals, Wayne State University and the Cuyahoga Metropolitan Housing Authority (CMHA), operating in both Cleveland and Detroit. The goal of ACHIEVE GreatER is to reduce cardiovascular complications and hospitalizations for Black patients living in public housing, and who are at risk of heart disease, by improving blood pressure, lipid, and glucose targets. The project, which seeks to determine the impact grassroots efforts can have to address cardiovascular health disparities, pays for community health workers, nurses and care coordinators to work with clients living in CMHA units and coordinate their care with clinical providers and other community resources. Better Health Partnership (BHP) is partnering with University Hospitals to offer a sustainable model for funding CHWs beyond the 5-year grant cycle via the Better Health Pathways HUB.
