“Like trying to weigh mice on a scale made for elephants.” This is the analogy that Professor David Rimm, a leading pathology expert at Yale School of Medicine, use to outline significant challenges in distinguishing between HER2-low and HER2-ultralow breast cancer using current IHC tests. Current diagnostic tests operate outside the appropriate dynamic range, making it difficult to detect lower HER2 levels. Rimm points out that these tests are not sensitive enough for accurate detection. Pathologists struggle to differentiate between HER2 0 and HER2 1+ disease, with current tests being only 80% accurate. MammaTyper® is quick, accurate, reliable and reproducible. It uses RT-qPCR technology which removes subjectivity and accurately measures ERBB2, ESR1, PgR and MKI67. The gold standard for diagnosis of skin cancer is dermoscopy which results in a limited diagnostic accuracy due to the complexity of visual inputs embedded in a dermoscopy image, and its dependency on physician skills. For example, in blinded tests dermatologists achieve at the lower end of human performance a mean sensitivity for melanoma detection of 40% and for more complex melanoma images detection is not better than chance. An AI decision support SaMD seems compulsory by these observations. Clinical tests should use real daily encounters, and not oversized skin lesions, more than 6 mm, a size you do not need AI assistance to decide.

I have developed an AI-powered Cancer Detection and Feedback System that allows users to upload skin lesion images via a user-friendly Streamlit interface. The system analyzes the uploaded image to predict whether the lesion is benign (non-cancerous) or malignant (cancerous). Once the prediction is made, the system provides detailed feedback based on the result: If benign, it offers an explanation of benign lesions, common examples like moles or freckles, and advises further consultation with a dermatologist for any concerns. If malignant, the system explains what a malignant lesion means, lists types of skin cancer (such as melanoma or basal cell carcinoma), highlights the importance of early diagnosis, and suggests next steps. The Potential of Radiologial 3D Assessments A recent study by Rabea Sobirey et al. in European Radiology highlights the effectiveness of 3D assessments in evaluating tumor response to immunotherapy in hepatocellular carcinoma (HCC) patients. Other than RECIST and mRECIST, the 3D quantitative enhancement-based response assessment tool qEASL demonstrated predictive power regarding overall survival and effectively identified non-responders.
