When excising a cutaneous malignancy, various techniques can remove the lesion and repair the surgical defect. Typically, an ellipse is drawn out after marking appropriate margins, the entire ellipse is removed, and a linear repair is performed. How ever, initially performing a round excision allows other repair techniques to be utilized depending on the body location. One technique involves removing Burow’s triangles to achieve a linear repair. This method can reduce scar length by 14%. Another technique is the purse-string closure, which is often used on the extremities and on the back of the hand. The potential benefits of a purse-string closure include a shorter and more cosmetically acceptable scar, but it is important to counsel patients on healing time. https://lnkd.in/dzPBf2ni

Percutaneous posterior sacroiliac joint fusion with intra-articular allograft for persistent left sacroiliac joint pain. The patient had the same procedure previously performed on the right side with >80% improvement in low back pain. While admittedly not as robust as metallic screws and implants, utilizing an intra-articular allograft may be used in situations where osteoporosis may prohibit use of traditional fixation screws, the patient cannot tolerate a more invasive procedure, or the patient wants to avoid hardware. An Epidural Steroid Injection (ESI) is a medical procedure commonly used to relieve pain and inflammation in the spine. It involves injecting a combination of a corticosteroid medication and a local anesthetic into the epidural space around the spinal cord and nerves. The primary purpose of an ESI is to reduce inflammation and alleviate pain caused by conditions such as herniated discs, spinal stenosis, or sciatica. The corticosteroid medication helps to reduce inflammation, which can relieve pressure on the nerves and decrease pain. The local anesthetic provides immediate pain relief by numbing the affected area. ESIs are typically recommended when conservative treatments such as rest, physical therapy, or oral medications have not provided sufficient relief.
