I'm pleased to share our team's recent review of diagnostic error in mental health. With so much of the field's current emphasis on expanding access to services, the conversation about quality and safety in mental health tends to be sidelined. I've seen discourse, mainly among psychotherapists, that questions the utility of diagnosis. While it's true that many therapeutic interventions are transdiagnostic, I challenge the idea that diagnosis is irrelevant, even taking into account the known flaws and limitations of our current nosology. Here are just a few examples of meaningful diagnostic errors I've personally seen in clinical practice: - Severe PTSD (multiple Criterion A traumatic events, flashbacks, dissociative symptoms) misdiagnosed as bipolar disorder - Delayed diagnosis of anorexia nervosa - Obsessive compulsive disorder misdiagnosed for years as unspecified "anxiety" - An adaptive response to intimate partner violence misdiagnosed as sexual dysfunction - A vascular defect in the brain misdiagnosed as sexual dysfunction - Substance use disorder misdiagnosed as schizophrenia While there are times that diagnostic uncertainty might indeed be a matter of "splitting hairs," there are other times when a missed, delayed, or wrong diagnosis has meaningful and sometimes life-altering consequences for our patients. I am not aware of any clinician (myself included) who has not made diagnostic errors. Pointing out the problem is not about blaming and shaming; rather, it's about calling attention to the pitfalls we all face in what is often a complex and dynamic diagnostic process, and what we might do about them. https://lnkd.in/gW84nuem
