According to CaringInfo.org, “Advance care planning is a process, not an event, and is planning for future care based on a person’s values, beliefs, preferences, and specific medical issues. An advance directive is the record of that process.” To learn more about #advancecareplanning, including the different forms included, practical tips for storage, and how to get started, visit the CaringInfo Advance Directives overview page: https://lnkd.in/ggmMR-xn. While geared toward the community and caregivers, providers will also find helpful tips for engaging in advance care planning. #NHDD

👏 Clinical systems need clinical input. Sounds obvious, but not always the case ... Great snippet below from our friends at Predictive Health Intelligence explaining why. We are proud to have played our part in the development of hepatoSIGHT, which uses data to identify individuals at risk of liver disease early. But, clinical input is absolutely integral to making the technology user friendly and intuitive. #liverdisease #publichealth #clinicalengagement
What do clinicians think of hepatoSIGHT? As a company, we were co-founded by a practicing NHS clinician, so we understand the pressures facing front-line healthcare workers. That's why clinical engagement has been at the top of our development agenda from day 1. We've created a tool that is intuitive, easy to use, and empowering. Listen to our Managing Director, Neil Stevens, and one of our collaborators Dr. Christina Owen discuss our approach and its impact. #ClinicalEngagement #MedicalTechnology #PublicHealth

As healthcare professionals, we're not just treating conditions—we're guiding patients through complex medical journeys. One of the most powerful tools in our arsenal is often overlooked: patient education. Think beyond the pamphlet. Real patient education is about empowerment. It's about giving our patients the knowledge and confidence to actively participate in their own care. An educated patient is more likely to adhere to treatment plans, experience fewer complications, and achieve better health outcomes. It's not just about informing—it's about transforming lives through knowledge. https://hubs.ly/Q02Lv_ZZ0 What strategies do you use to educate and empower your patients? I’ve been reflecting a lot recently on patient feedback both in the NHS and the private sector 🌟 Understanding the difference between digital clinical pathways and virtual care companions is crucial in providing holistic patient care. Digital Clinical Pathways are structured plans for treatment (often clinician or institution facing) In contrast, Digital or Virtual Care Companions offer clinical, educational and emotional support and assistance. As a patient, the feedback we get is that they get a sense of “virtually” being listened to or heard. Are both equally important or should the priority be patient-centric digital transformation? #morethanjustaPEP #healthcare #patientcare #clinicalpathways #virtualcarecompanion #Wellola
