What I'm reading: Drs. Jeffrey Millstein and Jeffrey Tokazewski discuss the challenges primary care physicians face managing their workloads when transitioning to value-based models. Making the shift to patient-first care can be time consuming and overwhelming, but physicians don't have to transition alone. Collaborative Health Systems' support teams provide data, streamline support and services (even telemedicine), and help shoulder administrative burdens so physicians can focus on providing timely access to high quality care. #valuebasedcare #primarycare https://lnkd.in/gCu2hn76

#NHSEngland's recent announcement to expand #VirtualWards is set to target improved patient flow and clinical outcomes. The expanded rollout will focus on managing conditions like frailty, #HeartFailure, and respiratory conditions, such as #COPD. These are critical areas where timely proactive intervention can make a significant difference in patient health and hospital efficiency. However, the focus of this initiative remains on acute short-term services. A crucial enabler for admission avoidance initiatives is supported self-management to empower proactive selfcare. This reduces the burden on care teams as part of a continuum of care focused on early identification and avoidance rather than short-term pre-acute and acute interventions. By integrating digital tools for self-management and long-term monitoring, multi-disciplinary teams can collaborate across settings, capturing and utilizing relevant data to provide high-quality integrated care. At Lenus, we're proud to support ICSs in scaling supported self-management integrated acute #VirtualWard services. Our suite of digital tools and advanced #AI technology helps identify and empower those at risk to help themselves manage their own condition, with a low burden on care teams. This streamlines #CarePathways, reduces #Admissions and ultimately improve #PatientOutcomes. Technology-enabled healthcare providers will deliver the most human-centric care our generation has ever witnessed. The irony is that the trend that has been slowly removing the human element from healthcare is the very same trend that will reverse this, making healthcare more human than it has ever been. What better way to start than at the entry point most utilized by patients – the Emergency Department. Want to learn more about how? ✍🏻 In our latest HealthBytes article we dive in deep, analyzing what can be done to optimize a patient’s ED experience, while simultaneously freeing up providers to deliver the empathetic care patients deserve. Innovation where IT simplifies processes for the end user which in turn helps to make everything easier, faster and more accurate is brilliant. Tech to solve real world problems instead of solutions looking for a problem is what I’m interested in.
WA's North Metropolitan Health Service (NMHS) recently launched the Emergency Care Navigation Centre, a transformative initiative at Sir Charles Gairdner Hospital. This marks a significant leap forward in emergency medicine, streamlining the process of assessing, treating, and referring patients within the Emergency Department (ED) waiting room, without replacing the essential role of medical staff. This, combined with other measures, has achieved a 48% reduction in ramping hours and notably improved patient flow. By facilitating access to patient information and enabling app-based self-registration, the system accelerates care delivery and allows clinicians to concentrate on high-quality emergency medicine. With plans to expand this technology to Fiona Stanley Hospital and beyond, Trier Medical's approach exemplifies how leveraging technological advancements can revolutionise healthcare, promising a more efficient and seamless experience for patients and clinicians across the nation. Congratulations to Trier Medical for leading the way in healthcare innovation, highlighting the importance of adopting such technologies to foster better patient care and system-wide improvements #buildbetter #reform #emergencydepartment
