Officially 9 months as a Behavioral Health Manager with evolvedMD & wanted to share a few 🔑 takeaways from my learning over the last few months... 🔑 OWN THE DATA: My Clinical Manager shared a piece of research with me during clinical supervision recently that really hit home. I literally keep it printed nearby as a visual reminder of how powerful collaborative care is and refer to it often. It involves a study of collaborative care vs. usual care: "Median time to remission was 86 days for the CCM group versus 614 days for the UC group". Patients can reach remission in 3 months within the collaborative care model vs. 20 months in usual care! Can you imagine the impact on a person's life? Check out the article: “Time to Remission for Depression with Collaborative Care Management”. 🔑 EVERY MINUTE COUNTS: The tradition of the 50-minute session originates from Freud (!). I have let go of the dusty old belief that there is magic attached to the 50-minute therapy framework. This has been liberating. Anecdotally I find that teens, young adults and people with health issues that impact stamina/focus/physical comfort often thrive within a shorter timeframe and mindfully incorporating 15-minute touchpoints throughout the month adds exponential support. 🔑 TEAMWORK MAKES THE DREAMWORK, still: Collaborative care feels similar to the hospice team approach that I LOVE so much-we work together, and everyone has a voice. When a patient's care involves their voice + input from the MA, PCP, Behavioral Health Manager, Clinic Manager, Care Coordinator and Psychiatric Consultant complex challenges can be approached with care, creative solutions unfold and can be carried out rapidly. Why do we consider the virtual vs. in-person preferences of all patients but those with mental illness and substance use disorders? Why do people push a single solution to fit all needs of someone with ACEs, depression or schizophrenia? These are rhetorical questions of course. It’s mostly bias, fueled by innovators and policymakers who tend to believe that they can only marshal enough resources for one major change (two more biases, of thought and resource allocation). There’s also significant ignorance still about the many options in mental healthcare - most people have never heard of a “crisis residential” facility or even 988. To them it’s all outpatient couch, Zoom, or pills. And if and when you destabilize (‘decompensate’ for you medical folks), most think it’s ER or out of luck. Over the last few years I’ve seen advocates and well-meaning leaders in the behavioral health space focus on only integrated BH care in primary care. Only virtual visit/digital startups. Only crisis residentials, only 988, etc. That’d be like saying you can ONLY have your PCP call you, an ambulance, OR a hospital visit if you have chest pain. Not all three as your heart disease progresses to a heart attack. 😔 I can count on one hand professional conversations (led by anyone but me) about patients with MHD/SUD requiring in-person AND telehealth options. That 45% of behavioral health patients did not believe their clinicians considered their preferences for virtual or in-person care and 32 said they did not receive their preferred method of treatment in this new RAND Health study should give you pause. Add it to the list of inequities where behavioral healthcare is behind and think about this next time you invest time or money into a singular solution. My prediction: wise startups and health system innovators will find a way and succeed clinically and financially, combining telehealth with brick and mortar care options. Those will be the last ones standing as unjust but real scarcity presses toward what matters. #mentalhealthishealth #choicesmatter

One of the main questions with increasing access to #mentalhealth assessment tools like the CAT-MH® is: what are the next steps to providing #treatment? Since the onset of the #covid19pandemic, one estimate shows that #telehealth-delivered behavioral health services have spiked 45-fold, which may provide one solution to the increasing demand for mental health services in the US. According to a Substance Abuse and Mental Health Services Administration (SAMHSA) advisory, telehealth enables #healthcare providers to deliver several behavioral therapies in a similar way to the same therapies delivered in an office setting. Video, audio, and asychoronous telepsyciatry methods allow healthcare providers to expand access to care for people in rural communities and in  areas where specialists are scarce. Telehealth can also help address the #stigma associated with seeking mental health services by increasing privacy, and enabling clinicians to provide more availability to patients with complex conditions.
