“From today, the policy of this department is that the NHS is broken.” Across the four decades I’ve been working in healthcare, I can’t ever remember a Secretary of State for Health & Social Care in the UK uttering these words as their first, opening policy statement. It’s utterly unflinching, candid and, as we all know, true. As the Secretary of State goes onto say: “When we were last in office, we worked hand in hand with NHS staff to deliver the shortest waits and highest patient satisfaction in history. We did it before, and together, we will do it again.” I was fortunate enough to play a part in the development of the first NHS Plan back in 2000 and believe it can be achieved again, albeit with tougher public finances and an equally pressing need to symbiotically sort social care too. Thanks Rhion Jones I agree. There is a nod to patient involvement but it’s limited. It doesn’t feel any more developed than 15 years ago. I think there is a missed opportunity to see the potential for engaging broadly and deeply with patients and communities as part of the solution. Not taking the problems and potential answers to people for their input - but genuinely laying out the problems for a more ground up view of the possible solutions. The report speaks of the social movement of staff but imagine if we open our eyes to the potential social movement of citizens! This takes a shift in mindset. The next stage of work on reforms will need to focus on HOW we achieve the aims in the report. We can’t do it without the people and there is room to build from here. With the likes of National Voices The Health Creation Alliance Centre for Population Health The Patients Association and miriad other independent organisations supporting particular health and care interests - this is possible! It starts with government seeing the power of a new kind of relationship with society around what health and care means as an act of shared endeavour. It needs brave thinking and being prepared to acknowledge that those in positions of authority don’t have all the answers.

Reactions to Lord Darzi’s Independent Investigation into the NHS have been mostly positive so I feel a bit of a party-pooper with this Blog. I argue that alongside the many reasons for its current disrepair (austerity/Lansley’s reforms/Treasury cap on capital/ COVID-19/No social care fix etc etc) there has been a failure of patient & public involvement. What I mean is that all the worthy people representing their communities have – like the rest of us – acquiesced in the decade-long decline of the service. Instead of being campaigners and advocates for MORE and BETTER, they have been reduced to defending the service from proposals for LESS and WORSE. Darzi is an optimist and thinks all is not lost provided Government and medics invest in the right things. But Top-DOWN decisions have not worked. We need far more BOTTOM-UP pressure. Without public support and public involvement, the chances of restoring the NHS to the state we need is slim. #consultation #consultationguru #NHS #LordDarzi #patientandpubicinvolvement #socialcare #Darzireport

When it comes to non-consultations (other than with introverted echo chambers of patients with chronic conditions and those who care for them) the NHS is in a league of its own. But how would one set about opening it up to public scrutiny when most of avoid unless we need it (i.e. fall ill, are injured or get old and frail). As part of my project to try to join up the dots at the local level with regard to community safety I chair the patient participation group at for our largest local practice and regularly receive attempts at "consultation". The quality has begun to improve over the past year but each time I get comments from other patients about the questions that should have been asked and what they want to say ... usually about the means of communicating with the practice and/or correcting errors in their records (including discrepancies between practice, local hospitals, the NHS app and the hospital apps).

Health care is always at the forefront of politics, it is the same in every country! The question is: Is our current system fit for purpose and if not how do we change it...? But not only from the political/funding point of view, but from the individual point of view - are we ready, as a society to play our individual part on a daily basis? We have enough evidence that smoking and drinking - kills, eating unhealthy - leads to various conditions, not exercising - contributes to poor health... So, are you ready to play your part? And once you do concurrently for 1 year - let's ask this questing 😉
