A patient is referred to hospice care when a doctor’s prognosis is six months or less and you might think that means that after 6 months care ends. The reality is that hospice care can continue as long as the need for hospice care remains. After the initial 6 month period, patient cases are reevaluated and as long as the patient still needs care, Community Hospice & Health Services will be there for you and your loved ones. #HospiceCare #EndOfLifeCare #PalliativeCare #HospiceSupport #HospiceServices #PatientCare #CaregiverSupport #CommunityHospice #QualityOfLife

Dispelling the Myths about Hospice My Physician Must Bring Up That I Am Ready For Hospice Care. Patients and their families and doctors realize hospice is appropriate, but that often happens later than it should. Patients and family members can initiate hospice care, they do not need to wait for doctors to suggest it because sometimes they won’t. For example, if patients are receiving treatments they don't want anymore, they or a caregiver can initiate hospice by discussing the option with their doctor. Another alternative is to contact Pathways Hospice directly to ask for an evaluation and determine qualification. Every illness has different indicators for when it might be time to begin hospice.
