Grade 3 - complete rupture of the ATaFL - anterior talofibular ligament. Ankle twist - 12 weeks ago: The patient is experiencing pain and swelling in the lateral ankle region. There is a complete discontinuity of the anterior talofibular ligament. Repair of acute lateral ligament tears is seldom recommended. Open repair does not seem to provide any advantages over closed management at the initial injury stage. For patients with chronic mechanical instability observed during clinical examination and functional instability, delayed repair may be necessary. In another recent KSSTA Journal publication, the second installment of our 3-part series, we shed light on how medial and lateral knee joint laxity varies with coronal deformity, proximal tibial angle and distal femoral angle. This study found increased medial ligament distractibility with increasing varus deformity. However, there was significant variability in ligamentous laxity within various limb alignment categories, suggesting that the anatomy and soft tissue identity of the knee is complex and highly variable. TKAs seeking to be more anatomic will not only need to restore alignment but also native soft tissue tensions.
