We have great stories to tell and this recent Listener article tells our Healthline and commitment to equity stories, very well. It was great to work with Donna Chisholm and talk about what a gamechanger telehealth has been, in particular for Māori healthcare, and to help explain how our Healthline nurses, paramedic and doctors work. “Telehealth isn’t the ambulance at the bottom of the cliff, but the trampoline, ‘bouncing’ patients to where they need to go.” Donna spoke with Whakarongorau CEO Glynis Sandland, teletriage nurse Helen, and Chief Clinical Officer Ruth Large (ia/she/her). What do you think? https://lnkd.in/gM_TRgZF Noting that while Healthline telehealth clinicians most often don’t see their patient, they do receive specific training on developing rapport over the phone, how to triage over the phone, and they bring all their clinical experience and training to each call. It is a specialist role. Like working in a virtual emergency department. They don't simply use a formula, or an algorithm. They do use a clinical decision support tool to help triage the call and build up a picture of what is happening for the patient.  And in some cases – as the article highlights – images and video can be shared by the patient, to further help with the triage.

Why is seeing a doctor so frustrating?? Waiting. Waiting to schedule appointments, talk to a doctor, or see the doctor once in the office plays an outsized role in frustration.   A recent The Wall Street Journal piece by Laura Landro addresses this common patient pain point and potential solutions. 🔴Root of the Waiting Problem: It's Getting worse nationally. 1/3 of Medicare patients are waiting more than a month to see doctors. In my state, specialty appointment wait can be greater than 6 months. The biggest reason for these waits is physician shortages. Federal data shows were short 13,000 primary care docs. Factors for physician shortages include burnout leading to a reduction in operating hours, financial forces causing doctors to switch to concierge models, and an aging physician population. There’s a deficit projection of 140,000 primary and specialty docs by 2036. Increasing healthcare consolidation causes extended waits for those out of network.   ✅ Ways to solve waiting:   Simpler scheduling: use calls, tests, and emails to schedule appointments and decrease hold times on the phone. Allow patients to schedule their appointments. Walk-in clinics: Leverage gaps in provider’s schedules due to cancellations to allow walk-in visits. Digital front doors: Portals that allow patients to fill out forms before visits. The existing ones are notoriously hard to use. Decreasing patient cycle time (time from entering office to leaving): earlier clinic starts, ensuring appropriate staffing at peak times, morning huddles to determine which patients might need extra orders, and the use of “ flow coordinators.” Flow coordinators are usually nurses or medical assistants who check on docs during visits to signal other patients are waiting. Team-based models: Leveraging nurse practitioners and physician assistants to see more patients. Brick and mortar are partnering with digital health companies:  Hackensack Meridian Health partnered with One Medical to build new clinics for in-person visits, with no wait times. Fixing patient portals: The complex user interface and the lack of provider time to answer messages have largely made patient portals irrelevant. Hopefully there’s a tech fix here, using AI. Compromised patient care, clinicians say More than a dozen doctors and nurses who work for the sprawling health system told Michigan Public and KFF Health News that patient care at its hospitals across the nation was compromised in the fallout of the cyberattack over the past several weeks. Clinicians working for hospitals in three states described harrowing lapses, including delayed or lost lab results, medication errors, and an absence of routine safety checks via technology to prevent potentially fatal mistakes. Despite a precipitous rise in cyberattacks against the health sector in recent years, a weeks-long disruption of this magnitude is beyond what most health systems are prepared for, said John Clark, an associate chief pharmacy officer at the University of Michigan health system. “I don't believe that anyone is fully prepared for a long-term process like this,” he said. Most emergency management plans he's seen “are designed around long-term downtimes that are into one, two, or three days.” Ascension in a public statement May 9 said its care teams were “trained for these kinds of disruptions,” but did not respond to questions in early June about whether it had prepared for longer periods of downtime.
