A Chiari malformation is a structural defect in the brain's cerebellum that causes it to bulge into the spinal canal through the foramen magnum, the opening at the base of the skull. Figure. On brain MRI of a person without Chiari malformation (CM) (left), the cerebellar tonsils (white arrows) do not descend below the level of the foramen magnum (yellow line). In a person with Chiari type I malforation (CM-I;right), the cerebellar tonsils (white arrow) descend more than 5 mm below the level of the foramen magnum (yellow line).
