Why do we perform home based respiratory care? Studies have shown that performing routine respiratory care at home not only helps patients breathe easier daily but decreases their long term need for hospitalizations with prophylactic practices. Giving daily therapy and performing routine trach care, cough assist, and suction not only improves a patient’s quality of life but also increases health, and decreases illnesses such as pneumonia. Additionally, it is cost effective to not only the patient but to our healthcare system. We help reduce the need for hospitalizations.

F695 Respiratory Care was the 7th most cited deficiency in standard health surveys so far for the 2024 Fiscal Year, being cited in 21% of surveys. F695 mandates that facilities must ensure residents requiring respiratory care, such as oxygen therapy, respiratory treatments, tracheostomy care, tracheal suctioning and mechanical ventilation, receive it according to professional standards, personalized care plans, and the resident’s preferences. Based upon the facility assessment and in consideration of the resident population, diagnoses, staffing/ staff qualifications, and facility resources, the facility must determine its ability to provide specialized respiratory care that meets the identified needs. I wanted to share this article for a couple of reasons. One, I am a trach fanatic, and whenever I see an article on trachs, I am usually excited. The safety feature of this drew me to the subject of decreasing pressure ulcers. But as I read and came to the pictures, my immediate thought was, "Who was in charge of these airways?". I have never truly seen trachs secured like this. I am happy with the pressure-reducing device; as you see from Fig. A-F , there is a noticeable difference in weight on the actual device. The authors are correct in that, as RTs, we all know how our devices hurt, and therefore, I think we should be in charge of every aspect of their care of them. Again, our devices hurt: Attaching anything to them causes pressure, especially to the trach. Adding an NG to the ETT or BIPAP mask adds pressure, we control these things. There were many great articles in RCJ this month, but I hope RTs read this one and especially look at the pictures. My only other comment is every RT should be on their hospital's SKIN team. The comment, "Furthermore, sometimes documentation falls under nursing and other times under respiratory therapy. As such, the outcome or ability to track down notations of tracheostomy-related pressure injury may be challenging due to this heterogeneity" bothered me. As RTs we document on our own airways. I could write a page more about PUs and skin https://lnkd.in/gDvfPkba but I believe this article speaks volumes in its own way.
